Purpose: To explore the value of systemic inflammatory markers as independent prognostic factors and the extent these markers improve prognostic classification for patients with inoperable advanced or metastatic gastric cancer (GC) receiving palliative chemotherapy. Methods: We studied the prognostic value of systemic inflammatory factors such as circulating white blood cell count and its components as well as that combined to form inflammation-based prognostic scores (Glasgow Prognostic Score (GPS), Neutrophil-Lymphocyte Ratio (NLR), Platelet Lymphocyte Ratio (PLR), Prognostic Index (PI) and Prognostic Nutritional Index (PNI)) in 384 patients with inoperable advanced or metastatic gastric cancer (GC) receiving first-line chemotherapy. Univariate and multivariate analyses were performed to examine the impact of inflammatory markers on overall survival (OS). Results: Univariate analysis revealed that an elevated white blood cell, neutrophil and/or platelet count, a decreased lymphocyte count, a low serum albumin concentration, and high CRP concentration, as well as elevated NLR/PLR , GPS, PI, PNI were significant predictors of shorter OS. Multivariate analysis demonstrated that only elevated neutrophil count (HR 3.696, p=0.003) and higher GPS (HR 1.621, p=0.01) were independent predictors of poor OS. Conclusion: This study demonstrated elevated pretreatment neutrophil count and high GPS to be independent predictors of shorter OS in inoperable advanced or metastatic GC patients treated with first-line chemotherapy. Upon validation of these data in independent studies, stratification of patients using these markers in future clinical trials is recommended. Source

Background: Weight loss in advanced gastric cancer (GC) has been widely acknowledged to be a predictor for poor survival. However, very few studies have investigated the weight loss that occurs during chemotherapy. Therefore, we focused on weight loss during chemotherapy in patients with advanced GC and investigated the concentrations of macrophage inhibitory cytokine-1 (MIC-1), which has been recognized as a probable etiological factor in anorexia and weight loss. Methods: We analyzed 384 patients with inoperable locally advanced or metastatic GC receiving first-line chemotherapy. Patients were assigned to one of two groups on the basis of their weight change during chemotherapy: >3% weight loss and ≤3% weight loss. Serum MIC-1 and C-reactive protein (CRP) concentrations were also assessed in these patients. Results: The >3% weight loss group had shorter overall survival (OS; 12.0 months vs. 17.5 months, P = 0.000) than the ≥3% weight loss group, and the survival rates improved if the weight loss was reversed during chemotherapy. Although the MIC- 1 concentrations were not correlated with weight loss before (P = 0.156) or during chemotherapy (P = 0.164), it correlated significantly with the CRP concentration (P = 0.001). Furthermore, elevated MIC-1 concentrations before chemotherapy (P = 0.017) and increased MIC-1 concentrations during chemotherapy (P = 0.001) were both found to be predictors of poor OS. Conclusions: Changes in the body weight during chemotherapy could influence the prognosis in patients with advanced GC, and the MIC-1 might be a potential predictive and prognostic biomarker in those patients. © 2014 Lu et al. Background: Weight loss during chemotherapy has not been exclusively investigated. Macrophage inhibitory cytokine-1 (MIC-1) might play a role in its etiology. Here, we investigated the prognostic value of weight loss before chemotherapy and its relationship with MIC-1 concentration and its occurrence during chemotherapy in patients with advanced esophageal squamous cell carcinoma (ESCC). Materials and Methods: We analyzed 157 inoperable locally advanced or metastatic ESCC patients receiving first-line chemotherapy. Serum MIC-1 concentrations were assessed before chemotherapy. Patients were assigned into two groups according to their weight loss before or during chemotherapy: >5% weight loss group and ≤5% weight loss group. Results: Patients with weight loss >5% before chemotherapy had shorter progression-free survival period (5.8 months vs. 8.7 months; p=0.027) and overall survival (10.8 months vs. 20.0 months; p=0.010). Patients with weight loss >5% during chemotherapy tended to have shorter progression-free survival (6.0 months vs. 8.1 months; p=0.062) and overall survival (8.6 months vs. 18.0 months; p=0.022), and if weight loss was reversed during chemotherapy, survival rates improved. Furthermore, serum MIC-1 concentration was closely related to weight loss before chemotherapy (p=0.001) Conclusions: Weight loss both before and during chemotherapy predicted poor outcome in advanced ESCC patients, and MIC-1 might be involved in the development of weight loss in such patients. Background: Most of the posterior inferior cerebellar artery (PICA) aneurysms arise at the vertebral arteryYPICA junction; distal PICA aneurysms are rare. Their clinical features and treatments are not fully understood. Methods: This was a retrospective study. We reported 9 patients with the distal PICA aneurysms treated by surgical clipping or endovascular embolization in our neurosurgical center. Results: All patients experienced sudden onset of headache with subarachnoid hemorrhage. The location of PICA aneurysms was at the anterior-medullary (n = 4), tonsillomedullary (TM) (n = 1), and telovelotonsillar (n = 4). Surgical clipping was performed in 3 and endovascular coil in 6 patients. All the PICA aneurysms were occluded with parent artery preservation, and all of the patients achieved good recovery. Conclusions: Distal PICAaneurysmis exceptionally rare; it could be treated successfully with surgical clipping or endovascular treatments. Copyright © 2012 by Mutaz B. Habal, MD. Source

Objective To investigate expression of Survivin and mucin 2(MUC2),and their correlation with atrophic gastritis with intestinal metaplasia and gastric carcinoma. Methods 30 normal gastric mucosa, 30 tissues with atrophic gastritis with intestinal metaplasia, 41 tissues with gastric carcinoma were detected with immunohistochemistry. The expressions of survivin and MUC2 in tissue microarray were studied. Results Two normal gastric mucosa expressed Survivin(7.14%),but MUC2 was not expressed in the normal gastric mucosa. The positive expression rate of survivin was 21.43% and MUC2 was 75.00% in atrophic gastritis with intestinal metaplasia. The positive expression rate of Survivin was 77.78%, MUC2 was 13. 89% in gastric carcinoma. The positive expression rate of Survivin and MUC2 in different gastric mucosa was of statistical significance(P <0.001). The expression of Survivin in gastric carcinoma showed statistically significant difference(P < 0.05) in different Lauren classification, depth of invasion and TNM categories. The expression of MUC2 in different Lauren classification, tumor differentiation was statistically significant difference(P<0.05). The expression of Survivin and MUC2 in gastric carcinoma was not associated with gender, patients age, lymph node metastasis, tumor location, tumor size(P< 0.05). The expression of Survivin shows no obvious correlation with MUC2(r =0. 215,P =0.209). Conclusion The expressions of survivin and MUC2 might play an important role in the development of gastric carcinoma. Source
