OBJECTIVE: To investigate the clinical significance of the three-step approach in laparoscopic hemihepatectomy.METHODS: A total of 32 patients received laparoscopic hemihepatectomy with the three-step approach in Yijishan Hospital affiliated to Wannan Medical College between Aug 2013 and Oct 2015. All patients underwent thin slice CT scan and hemihepatectomy was imitated with the imagic explorer, preoperatively. The vessel distribution was observed at the section and the three-step approach was used in the hemihepatectomy. Pre- and post-operative data were collected and analyzed retrospectively.RESULTS: The length of middle hepatic vein (MHV) was (59.1±12.9) mm and the number of branchs to the left and right lobe were 3.07±0.78 and 3.11±0.64 respectively. The distance between the first branch of MHV and the diaphragmatic surface was (28.07±3.74) mm and the distance between MHV and the visceral surface was (14.4±4.3) mm. The laparoscopic surgeries (left hemihepatectomy in 28 and right hemihepatectomy in 4) were performed successfully in all cases with the three-step approach, without any conversion to the open surgeries. The operation time was (165±42) min in left hemihepatectomy and (305±50) min in right hemihepatectomy. The intraoperative blood loss was (242±65) ml in left hemihepatectomy and (695±122)ml in right hemihepatectomy. All the patients recovered well without severe complications except for bile leakage in 3 patients who were cured with drainage. The postoperative hospital stay was (7.96±1.8) d.CONCLUSIONS: the distribution of vessel is regional at the section of hemihepatectomy and the three-step approach based on this feature is safe and effective in laparoscopic hemihepatectomy, which can shorten the operation time and reduce the difficulty of operation.

The aim of this study was to systematically evaluate the predictive value of time to positivity (TTP) in candidemia. All first episodes of candidemic patients admitted to our hospital between January 2008 and July 2012 were recorded retrospectively. We analyzed the relationship between TTP, identification of Candida species, antifungal agent susceptibility, and patients' clinical characteristics (30-day mortality, underlying diseases, and associated risk factors). TTP of simulated blood culture with equal inoculum amounts of different Candida species was determined. We included 87 patients during the study period, with a mean TTP of 43.47 ± 19.51 h. TTP of C. glabrata was significantly longer (p < 0.001) and TTP of C. tropicalis was significantly shorter (p < 0.001) than that of other Candida species. Receiver operating characteristic (ROC) curve analysis showed that TTP can predict C. glabrata (with the cut-off value of >45.17 h) and C. tropicalis (with the cut-off value of ≤33.17 h) in candidemia with good sensitivity and specificity. No statistically significant relationship was found between TTP, antifungal agent susceptibility, and patients' clinical characteristics (p > 0.05). TTP was not a risk factor associated with mortality (p > 0.05). The TTP result in simulated blood culture was in accordance with that of the included patients. TTP has been demonstrated to be helpful to differentiate C. glabrata and C. tropicalis from other Candida species in candidemia, and it is not associated with antifungal agent susceptibility and patients' clinical characteristics. TTP cannot predict pathogen concentration in the blood of candidemic patients. © 2013 Springer-Verlag Berlin Heidelberg.

To investigate the effect of standardized male circumcision with Shang Ring in reducing postoperative complications. We performed standardized male circumcision with Shang Ring for 351 males aged 4-58 (mean 31) years, 46 with phimosis and 305 with redundant prepuce, and observed the complications, including postoperative bleeding, infection, edema, and wound dehiscence. The main complications included infection (5 cases, 1.42%), mild edema (9 cases, 2.56%), moderate edema (5 cases, 1.42%), and wound dehiscence (6 cases, 1.71%), but no postoperative bleeding was observed. Using the standard protocol of circumcision with Shang Ring, well-trained male circumcision providers can significantly reduce postoperative complications and enhance the patient's satisfaction. Normalized training of the circumcision providers is essential for the popularization of standardized male circumcision with Shang Ring.


Intermittent Cyclic Mechanical Tension (ICMT) was applied to end plate chondrocytes by using an FX-4000T Flexercell Tension Plus unit (Flexcell International Corporation, Hillsborough, NC). Changes of end plate chondrocytes were observed after ICMT stimulation. To investigate the relationship between mechanical stimulation and calcification of end plate chondrocytes. Previous study showed that end plate calcification was related to mechanical stress, but there was no clear evidence to indicate whether or not mechanical stimulation could induce calcification of end plate chondrocytes in vitro. Rat end plate chondrocytes were cultured and ICMT (strain at 0.5 Hz sinusoidal curve at 10% elongation) was applied for 25 days, 4 hours a day and continued to culture for 5 days. End plate chondrocytes were incubated for 12 hours in the presence or absence of 10 ng/mL of transforming growth factor-β1 (TGF-β1) (prepared from a stock solution at 10 μg/mL in 2 mM citric acid containing 2 mg/mL bovine serum albumin) in MEM/F-12 containing a final concentration of 1% FCS. End plate chondrocytes calcification was stained by alizarin red S (AR-S). End plate chondrocytes viability was examined by LIVE/DEAD viability/cytotoxicity kit (Invitrogen, Carlsbad, CA). Related gene expression was examined by reverse transcription-polymerase chain reaction and Western blot. LIVE/DEAD assay verified that the nonloading (NC) group and the ICMT group end plate chondrocytes remained adherent, with no change in viability after the application of ICMT. Alizarin red staining showed that ICMT induced the calcification of end plate chondrocytes. Real-time reverse transcription-polymerase chain reaction showed that mRNA expression of endogenous TGF-β1 decreased and mRNA expression of type I, type X, osteocalcin and osteopontin increased after ICMT. The ankh gene expression of both mRNA and protein levels decreased in the ICMT stimulation. The ankh gene expression of both mRNA and protein levels increased in TGF-β1 stimulation. Compared with NC group, the alkaline phosphatase activities significantly increased in ICMT group. Our results directly showed that ICMT induced the calcification and downregulation of ankh gene expression of end plate chondrocytes, which may be caused by the endogenous TGF-β1.

To observe the effects of metformin on epididymal sperm quality and antioxidant function of the testis in diet-induced obesity rats. Thirty-two male SD rats were fed on high-fat food for 8 weeks to make obesity models, and another 8 were included as normal controls. Twenty-four of the model rats were equally randomized into a model control group to be fed continuously on high-fat food, a metformin group to be fed on normal food with metformin, and a normal food group. By the end of the 12th week, all the rats were killed for the determination of Lee's index, the organ coefficients of the testis and epididymis, epididymal sperm concentration, sperm motility, grade a + b sperm percentage, and the contents of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA) in the testicular homogenate. Lee's index was significantly increased in the model control group (P < 0.01) as compared with the other three. Lee's index was markedly higher in the normal control than in the metformin group (P < 0.05). The organ coefficients of the testis and epididymis were significantly decreased in the model control group (P < 0.01) as compared with the other three. Sperm concentration and motility and the percentage of a + b sperm were significantly decreased in the model control in comparison with the other three groups (P < 0.05 or P < 0.01). Sperm concentration was remarkably higher in the normal control than in the metformin and normal food groups (P < 0.05). The content of SOD (U/mg prot) was significantly lower in the model control (90.92 +/- 4.06) than in the normal control and metformin groups (101.69 +/- 8.49 and 102.04 +/- 10.67) (P < 0.05); that of GSH-Px (U) obviously higher in the normal control (28.32 +/- 2.28) than in the model control (23.49 +/- 1.08, P < 0.01), the metformin (25.73 +/- 2.14, P < 0.05) and the normal food group (25.77 +/- 2.19, P < 0.05), but evidently lower in the model control than in the metformin group (P < 0.05); and that of MDA (nmol/mg prot) significantly higher in the model control (2.68 +/- 0.76) than in the normal control (1.84 +/- 0.31, P < 0.01), the metformin (1.88 +/- 0.33, P < 0.01), and the normal food group (2.14 +/- 0.35, P < 0.05). Metformin therapy and improved diet could improve sperm quality and promote the antioxidant ability of the testis in diet-induced obesity rats.

To investigate the expression and clinical significance of miR-155 in the peripheral blood CD4(+);T cells of patients with allergic asthma. The expression of pre-mature BIC transcript and its processed microRNA miR-155 in the CD4(+); T cells from 50 patients with allergic asthma and 20 healthy individuals were measured using real-time PCR. The correlation of miR-155 expression and the disease severity of asthma was analyzed. The ability of miR-155 to influence Th1/Th2 differentiation was examined in vitro. The expression of pre-mature BIC and miR-155 in the CD4(+); T cells of asthma patients significantly decreased compared to those in normal controls (P<0.01). Furthermore, the expression of BIC in moderate and severe asthma patients was lower than that in normal controls (P<0.05). Among mild, moderate, severe asthma patients, BIC expression showed no statistical significance, but miR-155 expression of severe asthma patients was lower than that in mild asthma patients (P<0.05). The expression level of miR-155 was positively correlated to FEV1/pred(%) (P<0.01). miR-155 over-expression was found to promote CD4(+); T cells to Th1 differentiation, while of miR-155 was found to induce CD4(+); T cells to Th2 differentiation. The decreased expression level of miR-155 is correlated to asthma disease severity. It is suggested miR-155 plays an important role in development of asthma.

To express a chimeric gene R8 derived from the group 1 allergens of dust mites using prokaryotic expression system and detect their bioactivities. PCR amplification was performed using specific primers of Derf1 gene and the pUCm-T recombinant plasmid containing the R8 chimeric gene as a template. The PCR products were inserted into the pET28a(+) empty vector after double digestion using restriction endonuclease BamH I and Xho I, respectively. The recombinant plasmid was transferred into E. coli line BL21 and induced by 1 mmol/L isopropyl-beta-D-1-thiogalactopyranoside (IPTG). The expressed product was detected by SDS-PAGE and the target protein was purified. IgE binding assay of the purified protein R8 was detected by ELISA using dust mite allergic patient sera. For determining immunogenicity of R8 protein, 75 BALB/c mice were randomly divided into 5 groups, namely PBS (negative control), rDer f 1 group and rDer p 1 group (positive groups), R8 group and asthma group. The mice were treated with dust mite extract at 0, 7, 14 day by intraperitoneal injection of allergens (100 jl, 0.1 .tg/jl) and inhaled challenge as aerosol (0.5 microg/ml, 30 min/d) on day 21 for 7 days. Before inhalation in immunotherapy groups at 25-27 day, specific allergen immunotherapy was performed using rDerf 1, rDerp 1 and R8 allergens respectively. Mice in negative control group were treated with PBS all the time. Twenty-four hours after the last challenge, mice in every group were sacrificed. The bronchoalveolar lavage fluid (BALF) was collected. ELISA was used to detect the level of interferon-gamma (IFN-gamma) and interleukin 4 (IL-4) in BALF. SDS-PAGE analysis revealed that chimeric gene R8 was expressed with a band of approximately M(r) 35 000. Compared with groups of rDerf 1 and rDer p 1 [(80.44 +/- 15.50) and (90.79 +/- 10.38) microg/ml, respectively], IgE binding capacity of the protein R8 (37.03 +/- 12.46) microg/ml was statistically lower (P < 0.001). The level of IFN-gamma in sera of R8 group [(343.43 +/- 38.79) pg/ml] was higher than that of the PBS and asthma groups [(393.93 +/- 50.68) and (208.44 < or = 46.11)pg/ml, respectively] (P < 0.01), but no statistical difference to that of the rDerf 1 and rDer p 1 groups (P > 0.05). IL-4 level in R8 group was lower markedly than the others (P < 0.05 or P < 0.01). Chimeric protein R8 derived from the group 1 allergens of dust mites has been expressed with low allergenicity and high immunogenicity.

To observe the effects of electroacupuncture (EA) combined with Compound Salviae Miltiorrhizae Tablet (CSMT) on the expressions of brain derived neurotrophic factor (BDNF) and vascular endothelial growth factor (VEGF) in hippocampus CA1 of chronic cerebral ischemia (CCI) rats. Totally 50 Sprague-Dawley rats were randomly divided into the normal control group, the model group, the CSMT group, the EA group, and the EA +CSMT groups, 10 in each group. The CCI model was prepared by permanent bilateral common carotid arteries occlusion. One week after modeling, CSMT at 0.75 g/kg was given to rats in the EA + CSMT group and the CSMT group by gastrogavage, once daily, for 5 successive weeks. EA at Baihui (DU20) and Dazhui (DU14) was performed to rats in the EA group and the EA + CSMT group, lasting for 30 min, once daily, for 5 successive weeks. The expressions of BDNF and VEGF in the hippocampus CA1 area were detected by immunohistochemical assay and image analysis. Compared with the normal control group, the number of positively expressed BDNF and VEGF neurons and their expression intensity in the hippocampus CA1 of the model group obviously decreased (P < 0.01). Compared with the model group, the number of positively expressed BDNF and VEGF neurons and their expression intensity in the hippocampus CA1 of the CSMT group, the EA group, and the EA + CSMT groups obviously increased (P < 0.01, P < 0.05). The number of positively expressed BDNF and VEGF neurons and their expression intensity were obviously higher in the EA + CSMT group than those of the CSMT group and the EA group (P < 0.01). EA combined with CSMT could significantly increase the expressions of BDNF and VEGF in the hippocampus CA1 of CCl rats. Besides, their effects were significantly higher than those of the CSMT group or the EA group.


To investigate the relationship between ectonucleotide pyrophosphatase phosphodiesterase-1(ENPP-1) expression and transforming growth factor beta 1 (TGF-β1) of end-plate chondrocytes after stimulation with intermittent cyclic mechanical tension (ICMT) by using an FX-4000T Flexercell Tension Plus unit. Rat end-plate chondrocytes were cultured and ICMT (strain at 0.5 Hz sinusoidal curve at 10% elongation) applied for 7 days for 4 h/day and cultured for a further 2 days. End-plate chondrocytes were also exposed to 10 ng/mL of TGF-β1. Then, using small interfering RNA technology, small interfering TGF-β1 (siTGF-β1) was transfected. Expression of ENPP-1 and TGF-β1 was measured by real-time reverse-transcriptase polymerase chain reaction (RT-PCR) and western blotting. Expression of both ENPP-1 and TGF-β1 was up-regulated after ICMT. Both RT-PCR and western blot showed that ENPP-1 expression decreases with siRNA TGF-β1 after 3% elongation 40 min, and cultured for an additional 2 days. It was found that down-regulation of ENPP-1 gene expression induced by ICMT is likely dependent on TGF-β1 in end-plate chondrocytes. © 2013 Chinese Orthopaedic Association and Wiley Publishing Asia Pty Ltd.

To observe the effect of electroacupuncture (EA) on behavior reactions and hippocampal glucocorticoid receptor (GR) and mineralocorticoid receptor (MR) protein expression in rats with post-traumatic stress disorder (PTSD), so as to explore its mechanism underlying improvement of PTSD. Thirty male Sprague-Dawley rats were randomly divided into control, model and EA groups (10 rats/group). PTSD model was established by single prolonged stress. EA (2 Hz, 1 mA) stimulation was applied to "Baihui" (GV 20) and bilateral "Zusanli" (ST 36) acupoints for 30 min, once a day for a week. Morris water maze test was used to measure the ability of spatial learning and memory. GR and MR protein expression levels in the hippocampus tissee were examined with immunohistochemistry. Compared with the control group, the animals' escape latency and the expression level of hippocampal GR protein were increased significantly, and the expression level of hippocampal MR protein, as well the ratio of MR/GR expression were down-regulated considerably in the model group (P<0.05). In comparison with the model group, the animals' escape latency and the expression level of hippocampal GR protein were shortened pronouncedly, and the expression level of hippocampal MR protein, as well the ratio of MR/GR immunoactivity were up-regulated considerably (P<0. 05). EA of GV 20 and ST 36 can improve the learning-memory ability and regulate hippocampal MR and GR expression in PTSD rats, which may contribute to its effect in relieving PTSD in clinical practice.
