Reactions of alkyl acetoacetates, aromatic aldehydes, ammonium acetate, and 1,3-cyclohexanedione afford alkyl 4-aryl-2-methyl-5-oxo-1,4,5,6,7,8- hexahydroquinoline-3-carboxylates. Objective. To study melatonin secretion and its correlations with poststroke cognitive, emotional impairment and sleep quality in the acute period of stroke and to assess treatment efficacy of melaxen. Material and methods. We studied 96 patients with acute stroke. A battery of tests and scales for assessment of neurological deficit, neuropsychological status and emotional impairment was used. The night urinary level of 6-sulfatoxymelatonin was assessed. The relationship between 6-sulfatoxymelatonin and cognitive, emotional status and sleep parameters was analyzed. Results. The level of 6-sulfatoxymelatonin was decreased in the night urine. Patients with dysexecutive poststroke cognitive impairment had higher level of 6-sulfatoxymelatonin and patients with dysmnestic and mixed cognitive impairment had lower level of 6-sulfatoxymelatonin in comparison with patients with normal cognitive functions. Melaxen improved cognitive function and sleep parameters, reduced the level of anxiety in the early recovery period of stroke. Conclusion. A role of chronobiological processes in the development of clinical signs of stroke in the aspect of cognitive impairment is discussed.

Objective: To study chronobiological characteristics of ischemic stroke (II) and post stroke cognitive impairment (CI). Material and methods: We examined 80 patients with acute ischemic stroke. The cognitive status, sleep quality, chronobiological parameters and night urinary level of 6-sulfatoxymelatonin were assessed. The clinical observation of the efficacy of melaxen treatment in intensive care unit was performed. Results: The results of the study indicate the significance of social and environmental factors which determine the sleep-wake behavior in the development of pineal dysfunction. The pineal dysfunction probably underlies the development of stroke and accompanied neurodegenerative process. Conclusion: The use of melaxen in hyperacute stroke was associated with better recovery of circadian rhythms and rehabilitation results. © 2014, Media Sphera. All rights reserved.

Objective. To study the dynamics of plasma concentration of stress hormones (ACTH and cortisol) and lactate as well as the integrative stress level indicator at neuroreanimation stage of severe cranial-brain injury. Material and methods. Authors examined 34 patients with traumatic intracranial hematomas including 19 patients who died during the first two weeks after admission to the hospital and 15 patients who were discharged for outpatient treatment. To assess stress level, we used the indicator of stress level estimated as body mass x 1/3 x heart rate frequency x pulse pressure x 0,000126. Contents of blood plasma ACTH and cortisol were determined using radioimmune method, lactic acid was measured with enzyme colorimetric method. A control group included the indicators of 10 healthy people aged from 24 to 44 years. Results and conclusion. In the first 24 h, the overstrain of the neuroendocrine system in response to strong stress influence of the trauma was noted in all patients. In patients with fatal outcome, an increase in cortisol in the first 24 h was excessive and lactate level was significantly higher during the observational period compared to the patients with favorable outcome. The peak of mortality was at 1—5 days when the stress level indicators, contents of ACTH, cortisol and lactate reached maximal values indicating the stress-induces overstrain of the hypothalamic- pituitary-adrenal axis and promoting the development of brain tissue hypoxia. © 2015, Media Sphera.
