In the course of homogenizing annealing of aluminum alloys prepared by continuous or semi-continuous casting, chemical inhomogeneity is reduced within individual dendrite cells (crystal segregation). This is almost a diffusion process proceeding at a temperature close to the liquid material temperature. In this process there is transfer of soluble intermetallic compounds and eutectics into solid solution, which considerably slows down crystal segregation. The temperature, duration of homogenization, dendrite size, and diffusion time affect homogenization. This article is devoted to optimizing homogenization with respect to its duration and homogenizing annealing temperature, which affect the mechanical properties of the Al-Zn5.5-Mg2.5-Cu1.5 alloy. © 2011 Springer Science+Business Media, Inc. Background. The antinociceptive effect of intrathecal midazolam is based on its affecting spinal gamma-amino butyric acid receptors. Objective. To evaluate pain relief in patients with chronic low back pain and failed back surgery syndrome after a single-shot intrathecal administration of midazolam. Design. A prospective, open-label study. Outcome Measures. The analgesic effect was determined using a patient questionnaire during subsequent visits to the pain therapy service. We classified at least a 50% pain reduction with improved quality of life and improved functional condition as a positive outcome. Results. Between 1995 and 2010, we performed 500 administrations: 227 administrations in 57 male patients and 273 administrations in 69 female patients. We performed 81 administrations for chronic low back pain and 419 administrations for failed back surgery syndrome. The average age of our patients was 50 years (range 28 to 86). The dose administered ranged from 2 to 5mg of midazolam. The analgesic effect lasted 9.7 weeks on average, ranging from 1 week to 3 years; the most common reported duration was between 4 and 12 weeks (3 months). In 65% of patients, we achieved pain relief lasting 4 weeks or longer; in 13%, the administration provided no analgesic effect at all. The incidence of side effects (drowsiness, nausea, headache, or transient worsening of complaints) was rather low. Conclusion. Intrathecal midazolam is a useful supplement to standard analgesic therapy with opioids, non-opioids, or spinal steroids. Wiley Periodicals, Inc. Source

Introduction: Injuries to the distal tibiofibular syndesmosis are frequent and continue to generate controversy. Methods: The majority of purely ligamentous injuries (“high ankle sprains”) is not sassociated with a latent or frank tibiofibular diastasis and may be treated with an extended protocol of physical therapy. Relevant instability of the syndesmosis with diastasis results from rupture of two or more ligaments that require surgical stabilization. Syndesmosis disruptions are commonly associated with bony avulsions or malleolar fractures. Treatment consists in anatomic reduction of the distal fibula into the corresponding incisura of the distal tibia and stable fixation. Proposed means of fixation are refixation of bony syndesmotic avulsions, one or two tibiofibular screws and suture button. There is no consensus on how long to maintain fixation. Both syndesmotic screws and suture buttons need to be removed if symptomatic. Results/Complications: The most frequent complication is syndesmotic malreduction and may be minimized with open reduction and intraoperative 3D scanning. Other complications include hardware failure, heterotopic ossification, tibiofibular synostosis, chronic instability and posttraumatic arthritis. Conclusion: The single most important prognostic factor is anatomic reduction of the distal fibula into the tibial incisura. © 2014, Springer-Verlag Berlin Heidelberg. Source

We describe the isolation of an NDM-1-producing Acinetobacter baumannii in a Czech patient repatriated in July 2011 from Egypt. The infection spread to another patient on the same ward. Both isolates showed the same resistance pattern and were susceptible only to colistin. They had an identical PFGE pattern and belonged to the same sequence type ST 1. Sequencing of the bla NDM gene identified the NDM-1 variant of the carbapenemase, surrounded by two copies of insertion sequence ISAba125. Source

Objective: The author focuses on the analysis of the surgical technique blepharoplasty of upper and lower eyelids. Based and draws from his own experience, when he was allowed in the period 2005-2015 at an eye clinic and MEDILAX clinic personally perform 2904 blepharoplasties of the upper eyelids, and 1452 lower eyelid blepharoplasties in a total of 3257 patients. Methods: This is a retrospective set of 3257 patients, who underwent in 2005-2015 upper blepharoplasty surgery (2904) or lower eyelids (1452). The surgery was performed in all patients under local anesthesia. Postoperative monitoring is planned for 3 weeks, 6 weeks and 3 months after surgery. The total number of patients outnumber men at a ratio of 85% to 15%. In the last 5 years from (2010 to 2015, set of 1817 patients) preoperative subjective complaints of patients were recorded in preoperative preparation. Specifically, it was a feeling of heavy, tired eyelids, increased lacrimation and recurrent swelling of the eyelids. The intention of monitoring is a subjective assessment of the improvements of observed parameters in patients after 3 weeks, 6 weeks and 3 months after surgery. Results: The results obtained after evaluating patient response we confirm the positive effect not only on the aesthetic plane of surgery, but the surgery will significantly and positively influences the function of the eyelids and subjective feelings operated. All patients improved monitored parameters. The greatest benefit at 3 months after surgery were at the upper eyelid at 96.3% relief from the weary eyelids, tearing relief in 41.4%, with the lower lids of 94.2% disappearance of recurrent swelling of the eyelids. Conclusion: The analysis confirms the positive impact of blepharoplasty on subjective complaints of the patient on an objective aesthetic and functional parameters. In the interest of the patient and in the interest of our operations is a positive result and mutual satisfaction while minimizing risk. If the patient has unrealistic ideas, and yet calls for performance that is not medically feasible, it is advisable that the patient is refused. Source
