Objective: To assess the feasibility of patient-controlled intravenous analgesia (PCIA) for advanced cancer patients with severe pain in primary hospitals. Methods: Morphine PCIA was applied to 27 advanced cancer patients with poor analgesic effect by three-step analgesic ladder therapy. Numeric rating scales (NRS) score, pain relief rate, first pain relief time, average dosage and adverse reactions were observed. Results: In 27 patients, the pain relief rate was 92.6%, the average first pain relief time was 34.1 min, NRS score decreased from 8.75±1.34 to 1.61±2.32 after treatment of PCIA (P<0.05), and adverse reactions were mild. Conclusion: Morphine PCIA can be used for advanced cancer patients with severe pain in primary hospitals as the replacement of three-step analgesic ladder therapy. ©, 2015, Journal of Practical Oncology, Editorial Board. AIM: To explore the efficacy and safety of mitomycin C used in trabeculectomy by analyzing intraocular pressure and blood flow. METHODS: A total of 103 patients with glaucoma were divided into 2 groups randomly, and all patients were treated with trabeculectomy, while patients in the observation group were given mitomycin C extra. At 6~12mo follow-up were completed after operation, and the clinical efficacy, complications and hemodynamic characteristics of central retinal artery were compared between groups. RESULTS: The clinical efficacy of the observation group was higher than that of the controls (P<0.05). There was no significant difference in complication rate between groups (P>0.05), but the complication type was differentiate to some extent. Low intraocular pressure and photophobia symptoms occurred mostly in the observation group, while cicatricial obstruction occurred mostly in the controls. Before treatment, there was no significant difference in hemodynamic characteristics between groups (P>0.05). One month after treatment, the peak systolic velocity (PSV) and end diastolic velocity (EDV) of the observation group were obviously lower, while resistance index (RI) and pulsatility index (PI) were higher than those of the controls, the difference was statistically significant (P<0.05). Six months after treatment, the PSV and EDV of the observation group were obviously higher, while RI and PI were lower than those of the controls, the difference was statistically significant (P<0.05). CONCLUSION: Mitomycin C has a short-term harmful effect on glaucoma patients after trabeculectomy, but its long-term effect is safe and reliable. © 2015 by the IJO Press.

AIM: To evaluate the effect of trabecular filtration surgery combined with scleral interlayer filled with silicon rubber for refractory glaucoma and its complications. METHODS: A total of 32 cases (32 eyes) of refractory glaucoma underwent trabecular filtration surgery combined with sclera interlayer filled silicone piece, namely trabeculectomy combined silicon rubber filled in sclera between layers was as experimental group. Twenty-one cases (21 eyes) treated by simple trabecular filtration surgery were as control group. Reducing effect of intraocular pressure (IOP) and complications of two groups were observed after operation. RESULTS: Postoperative follow-up was 6-24 months. The mean IOP was 14.36±5.2 mmHg (1 mmHg=0.133 kPa) in experimental group and 23.36±9.3 mmHg in control group after operation. IOP of three eyes (9.4%) in experimental group and 5 eyes (23.8%) in control group was over 30 mmHg. There was no serious complications and rejection correlated with silicone piece during the follow-up period. CONCLUSION: Silicone piece filled between sclera layer is an effective method for refractory glaucoma, and can effectively reduce IOP. Due to its economic, convenient, especially can replace expensive imported drainage plant figure treatment of refractory glaucoma in our country the grass-roots hospital, it should be widely applied.

AIM: To investigate the long-term effect of different incisions on the tear film after phacoemulsification. METHODS: Sixty-four patients (76 eyes) without dry eyes were selected and received phacoemulsification. The patients were divided into two groups randomly. Group A 30 patients (38 eyes) was performed with a 2.5 mm mini-incision coaxial phacoemulsification and group B 34 patients (38 eyes) was performed with a sub 1.8mm micro-incision cataract surgery (MICS). Schirmer I test (SIt), tear break-up time (BUT), corneal fluorescein staining (CFS) and tear osmolarity were observed at 3d preoperatively and 1wk, 1, 3, 6 mo and 1a postoperatively. The results were analyzed using a chi-square test and t-test with SPSS 19.0. RESULTS: (1) Schirmer I test (SIt): there was a significant increase in SIt at 1wk postoperatively. The difference between group A and group B was significant (P<0.01) at 1 wk postoperatively, but it was insignificant at other postoperative time points (P>0.05). (2) Tear break-up time (BUT): there was a large reduction in BUT at 1 wk, 1, 3 mo postoperatively. The difference between group A and group B was significant (P<0.05). However, there were no significant differences between the two groups at other times (P>0.05). (3) CFS score: there was a large increase in CFS at 1 wk, 1, 3 mo postoperatively. The difference between group A and group B was significant (P<0.05). However, there were no significant differences between the two groups at other times (P>0.05). (4) Tear osmolarity: there was a large increase in tear osmolarity at 1wk, 1mo postoperatively. The difference between group A and group B was significant (P<0.01). However, there were no significant differences between the two groups at other times (P>0.05). CONCLUSION: The stability of tear film in patients underwent sub 1.8 mm micro-incision cataract surgery(MICS) is much better than in patients received 2.5 mm mini-incision coaxial phacoemulsification, but symptomys of dry eye in patients operated with MICS are much more serious during the early postoperative period. There is no significant difference in the long-term effects on tear film between the two types of incision. Copyright 2014 by the Press.

OBJECTIVE: To investigate the association between androgenetic alopecia (AGA) and metabolic syndrome (MS).METHODS: Literature on association between AGA and MS up to December 26, 2013 was searched from PubMed, Web of Knowledge, Scopus, Cochrance library, SinoMed, CNKI, Wanfang and VIP databases, and the studies met the eligibility criteria were selected. Meta-analysis was performed by using StataSE 12.0 software to determine the association between AGA and MS.RESULTS: Four case-control studies and 2 cross-sectional studies met the eligibility criteria, including 950 AGA subjects and 3056 control subjects were entered the analysis. Meta-analysis showed that AGA was significantly correlated with MS (OR=2.70, 95%CI: 1.67-4.37, P<0.01). Stratification analysis showed that AGA was significantly correlated with MS in male (OR=2.30, 95%CI: 1.33-3.98, P<0.01) and female subjects (OR=4.61, 95%CI: 1.26-16.94, P<0.05); and AGA was significantly correlated with MS in European (OR=5.29, 95%CI: 2.86-9.80, P<0.01) and Asian subjects (OR=1.92, 95%CI: 1.18-3.10, P<0.01).CONCLUSION: Based on the available data, AGA may be a risk factor for MS, indicating that AGA patients would be a targeting population for screening of metabolic syndrome.

OBJECTIVE: To explore the anticancer mechanism of aqueous extract of Taxus Chinensis (Pilger) Rehd (AETC). METHODS: The serum pharmacological method was used to avoid interference from administration of the crude medicinal herbs. Eight purebred New Zealand rabbits were used for preparation of serum containing various concentrations of AETC. Forty- eight Balb/c-nu mice were used for in vivo experiments. The effects of serum containing AETC on the proliferation of A549 cells and expression levels of the epidermal growth factor receptor/mitogen- activated protein kinase (EGFR/MAPK) pathway- related proteins in vitro were investigated. Additionally, the effects on the growth of A549 xenografts in nude mice, and expression levels of the EGFR/ MAPK pathway-related proteins in the xenografts, were investigated. RESULTS: 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide (MTT) assay revealed that the serum containing AETC significantly decreased the viability of A549 cells in a dose-dependent manner. Western blot showed that the serum containing various concentrations of AETC strongly reduced the levels of phospho-Jun N-terminal kinase (p-JNK) and phospho-extracellular signal-regulated kinase1/2 (ERK1/2) while it increased the level of p-p38. However, no significant effects on the expression levels of JNK, ERK1/2, and p38 MAPK were found. In addition, an anticancer effect from AETC was observed in vivo in the Balb/c-nu mice bearing A549 xenografts. CONCLUSION: AETC has significant effects on the growth of A549 xenografts and on the activity of the EGFR/MAPK pathway. Therefore, AETC may be beneficial in lung carcinoma treatment. © 2014 JTCM. Objective: To study the correlative factors of neurotoxicity associated with oxaliplatin (L-OHP). Methods: Sixty-four cases of gastric cancer and intestinal cancer after radical resection who used L-OHP and 5-FU/CF chemotherapy were collected in the study. At the end of the sixth cycle, the grade of neurotoxicity, gross of L-OHP, interval time of L-OHP, patient age, sex, stage and position of tumor and toxic reaction were recorded. Results: All the factors were analyzed with Logistic regression model (α =0.05), six factors were identified in the univariate analysis, including patient age, gross of L-OHP, interval time of L-OHP, arrest of bone marrow, gastrointestinal reaction and hepatic dysfunction. The results of Logistic multivariate analysis (α =0.05) indicated that patient age, gross of L-OHP, interval time of L-OHP and arrest of bone marrow were the correlative factors. Conclusion: The dosage of L-OHP should be regulated to alleviate oxaliplatin-related neurotoxicity based on the patient age and other factors.

AIM: To evaluate the relationship between clinical tests (traditional and new) [lid-wiper epitheliopathy (LWE), lid-parallel conjunctival folds (LIPCOF)] and dry eye symptoms after 3mo of phacoemulsification. The combination of these tests can improve predictive ability for the development of dry eye symptoms. METHEDS: Tear film was monitored in 43 patients (43 eyes) afterphacoemulsification. A new clinical test was used to evaluate the dry eye symptoms after surgery. Tear meniscus height (TMH), LIPCOF, phenol red thread test (PRTT), corneal and conjunctival staining and LWE grades were observed. The results were analyzed by using SPSS 19.0. Symptoms were assessed by using the ocular surface disease index (OSDI). RESULTS: LWE was significantly correlated to nasal nasal and temporal side LIPCOF (P<0.05). LIPCOF and LWE were significantly correlated to NIBUT and PRTT (P<0.01). Significant correlations were found between TMH and PRTT (P<0.01). OSDI scores were significantly correlated to TMH, PRTT, LIPCOF and LWE (P<0.05). Significant discriminators of OSDI+/- were nasal LIPCOF [area under the receiver operating characteristic curve (AUC) (0.803), TMH (0.706), PRTT (0.755), LIPCOF (temporal/sum, 0.725/0.763) and LWE (0.727), etc]. Best predictive ability was achieved by nasal LIPCOF. CONCLUSION: The individual tests TMH, PRTT, LIPCOF and LWE were significantly but moderately related to OSDI scores.
