Background: The resistance of Helicobacter pylori (H. pylori) to antibiotics is increasing worldwide, lowering its efficacy in current eradication therapies. This study evaluated H. pylori resistance to antibiotics in the southeast coastal region of China and suggests appropriate alternatives. Materials and Methods: Seventeen thousand seven hundred and thirty one H. pylori strains were collected from eight areas of two provinces in coastal southeast China from 2010 to 2012. The resistance of these strains to six antibiotics was tested using the agar dilution method. Results: The resistance rates to clarithromycin, metronidazole, levofloxacin, amoxicillin, gentamicin and furazolidone were 21.5, 95.4, 20.6, 0.1, 0.1 and 0.1%, respectively. Double, triple and quadruple antibacterial resistant percentages were 25.5, 7.5 and 0.1%, respectively. A positive association between the resistance to levofloxacin and to clarithromycin was found, but there was a negative correlation in the resistances to levofloxacin and to metronidazole. Conclusions: The prevalence of H. pylori resistance to clarithromycin, metronidazole, levofloxacin and multiple antibiotics in coastal southeast China is high. Choice of therapy should be individualized based on a susceptibility test in this region of the country. © 2013 John Wiley & Sons Ltd.

To study the expression and significance of hypoxia-inducible factor -1 alpha (HIF-1α) in prostate cancer, successfully established recombinant expression plasmids pcDNA3.1-HIF-1α and pll3.7-siHIF-1α were transfected into PC-3 prostate cancer cells using Fermentas transfection reagents. Stable HIF-1α expression cell lines were selected using G418 selective culture. Protein and mRNA expressions of HIF-1α, MMP-2 and MMP-9 were assayed by Western blot and RT-PCR. Cell growth was evaluated using the MTT assay growth curve. The ability of invasion of prostate cancer cells was assayed in Transwell chambers. Compared to other four groups, HIF-1α mRNA expression was not obviously upregulated in pcDNA3.1-HIF-1α-PC-3 cells, while it was obviously downregulated in pll3.7-siHIF-1α-PC-3 cells. MMP-2 and MMP-9 mRNA were significantly upregulated in pcDNA3.1-HIF-1α-PC-3 cells. HIF-1α, MMP-2 and MMP-9 protein were upregulated in pcDNA3.1-HIF-1α-PC-3 cells by Western blot, while they were downregulated in pll3.7-siHIF-1α-PC-3 cells. The abilities of proliferation and invasion of prostate cancer cells were statistically significantly enhanced in pcDNA3.1-HIF-1α-PC-3 cells (P 0.05). Proliferation and invasion of PC-3 prostate cancer cells are promoted by HIF-1α, which provides a foundation for the research on HIF-1α in the pathogenesis of prostate cancer and suggests that HIF-1α gene may be a good target for anti-tumor therapy. © 2011 Academic Journals.

Intestinal tuberculosis (ITB) and Crohns disease (CD) are important differential diagnoses that can be difficult to distinguish. Polymerase chain reaction (PCR) for Mycobacterium tuberculosis (MTB) is an efficient and promising tool. This meta-analysis was performed to systematically and objectively assess the potential diagnostic accuracy and clinical value of PCR for MTB in distinguishing ITB from CD.We searched PubMed, Embase, Web of Science, Science Direct, and the Cochrane Library for eligible studies, and nine articles with 12 groups of data were identified. The included studies were subjected to quality assessment using the revised Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool.The summary estimates were as follows: sensitivity 0.47 (95% CI: 0.42-0.51); specificity 0.95 (95% CI: 0.93-0.97); the positive likelihood ratio (PLR) 10.68 (95% CI: 6.98-16.35); the negative likelihood ratio (NLR) 0.49 (95% CI: 0.33-0.71); and diagnostic odds ratio (DOR) 21.92 (95% CI: 13.17-36.48). The area under the curve (AUC) was 0.9311, with a Q* value of 0.8664. Heterogeneity was found in the NLR. The heterogeneity of the studies was evaluated by meta-regression analysis and subgroup analysis.The current evidence suggests that PCR for MTB is a promising and highly specific diagnostic method to distinguish ITB from CD. However, physicians should also keep in mind that negative results cannot exclude ITB for its low sensitivity. Additional prospective studies are needed to further evaluate the diagnostic accuracy of PCR.

AIM: To evaluate the efficacy of perioperative application of comfort nursing in patients with gallstone disease undergoing endoscopic retrograde cholangio-pancreatography (ERCP), influencing factors, and the impact on the quality of nursing care. METHODS: From January 2015 to December 2015, 168 patients who underwent ERCP were randomly divided into two groups according to the time of admission. An intervention group (n = 106) received comfort nursing, and a control group (n = 62) received routine nursing. Sleep quality, intraoperative abnormal fluctuation of blood pressure, intraoperative suspension, postoperative food intake without permission, patient satisfaction, postoperative recovery, and postoperative incidence of pancreatitis were compared between the two groups. Logistic regression analysis was used to identify independent factors related to comfort nursing. RESULTS: There was no significant difference in gender, age, educational level or occupation between the two groups (P > 0.05). Sleep quality, intraoperative abnormal fluctuation of blood pressure, intraoperative suspension, postoperative food intake without permission, and patient satisfaction were significantly improved in the intervention group compared with the control group, but postoperative recovery and postoperative pancreatitis showed no significant difference between the two groups. Sleep quality, intraoperative abnormal fluctuation of blood pressure, intraoperative suspension, postoperative food intake without permission, and patient satisfaction were factors independently associated with comfort nursing. CONCLUSION: Comfort nursing can improve sleep quality and patient’s mood fluctuation, reduce the consumption of food without permission, and improve patient satisfaction in patients with gallstone disease undergoing ERCP. © 2016 Baishideng Publishing Group Inc. To compare the long-term influence of vertebral fixation through or across the affected vertebra on vertebral morphology. Clinical data of 48 patients with simple thoracic and lumbar spinal fractures who were admitted between Jan. 2008 and Dec. 2010 were analyzed retrospectively. Among them 36 cases (28 males and 8 females) were fixed through the injured vertebra (group A) and 12 cases (8 males and 4 females) were fixed across the injured vertebra (group B). All patients were followed up for 6-36 months (mean 11.5 months). The vertebral body height, endplate angle and neurofunction were compared between the two groups before surgery, a week after surgery and at the end of the follow-up period. There was no statistically significant difference in vertebral body height,endplate angle and neurofunction before operation between group A and B (P > 0.05). Vertebral body height and endplate angle improved in both groups a week after operation and at the end of the follow-up period as compared with those before operation (P < 0.05), and the efficacy in group B was significantly better than that in group A (P < 0.05). There was no significant difference in neurofunction between the two groups (P > 0.05). The fixation method through the injured vertebra had a better reduction effect, more stable fixation, and a better long-term effect on vertebral morphology than that across the injured vertebra in the treatment of thoracolumbar vertebral fractures.

Background: As current symptomatic treatments of constipation are still unsatisfactory, an increasing number of patients seek help from Chinese medicine (CM), particularly Chinese herbal medicine (CHM). This study aimed to review the most frequently used CHM herbs and formulae, proprietary CHMs, and herb-drug interactions for functional constipation using zheng (syndrome)-based differentiation, and to determine the current practice of zheng-based CHM treatments for functional constipation. Methods: We developed a search strategy to include all the related clinical studies of CHM for constipation and set inclusion and exclusion criteria as studies on subjects with constipation of all ages and both sexes, using objective measures from laboratory or imaging techniques. The interventions included single herbs, CM classical formulae, CM new formulae, and Chinese herb-derived products and combination products. The clinical study types included were quasi- or randomized controlled trials, observational clinical studies, case series or case reports, and other types of appropriate research methods. The data concerning study design, sample size, mode of recruitment, sampling and diagnostic procedure, inclusion and exclusion criteria, and participants' characteristics (including age, sex, and duration of constipation). CM patterns, CM treatment principles, treatment regimen, and CM treatment outcomes were recorded. Results: A total of 29,832 relevant records were found, of which 8541 were duplicate records and 20,639 were excluded for reasons of irrelevance. The full text of 965 articles was retrieved for detailed assessment, following which 480 articles were excluded for various reasons. From the included articles, we retrieved 190 different CM zheng diagnoses from 485 individual studies. The most common zheng was dual deficiency of qi and blood (N=48), which was diagnosed in 948 out of 15,740 subjects. The most frequently used classical formula was Ma-Zi-Ren-Wan (MZRW) (N=75) and the most frequently used proprietary CHM was Run-Chang-Wan (N=87). The most frequently used combined medication was Da Huang with sodium bicarbonate tablets (frequency across all studies, n=23), followed by Fan Xie Ye with lactulose oral solution (n=8), Ma-Ren-Ruan-Jiao-Nang with lactulose oral solution (n=6) and Liu-Wei-An-Xiao-Jiao-Nang (n=6) with mosapride citrate tablets. Conclusion: This study examined the use of CHM for constipation and summarized the herbs, formulae, proprietary medicines, and herb-drug interactions application. These data indicated there were limited information about herb-drug interactions and adverse effects of CHM and further randomized controlled trials with strict design are necessary. © 2016 Zhong et al.

AIM: To investigate the relationship between CD151 and Integrin α6 expression and clinicopathological parameters in colorectal cancer, and analyze the role of the two proteins in epithelial-mesenchymal transition (EMT). METHODS: Immunohistochemistry was performed to detect the expression of CD151, Integrin α6, E-cadherin and Vimentin in 132 colorectal cancer specimens using tissue microarray. RESULTS: The positive rates of CD151 and Integrin α6 in colorectal cancer tissues were 65.9% and 75.7%, respectively, significantly higher than those in tumor-adjacent normal tissues. CD151 and Integrin α6 expression was closely related with the depth of invasion and lymph lode metastasis, while the expression of CD151 was closely associated with low E-cadherin expression and high Vimentin expression. CONCLUSION: CD151 may accelerate the invasion and metastasis of colorectal cancer by promoting EMT. © 2015 Baishideng Publishing Group Inc. All rights reserved.

Erectile dysfunction is a common complication following cryotherapy for prostate cancer. In this study, we investigated the efficacy of adipose-derived stem cells (ADSC) in improving erectile function in a rat model of cavernous nerve (CN) cryoinjury and the possible mechanisms. Male rats were intracavernous (IC) injected with EdU-labeled ADSC after bilateral CN cryoinjury. Penile tissues were harvested for histology and protein level detection at 1 and 4 weeks after ADSC administration. Erectile function was assessed prior to tissue harvest. We found that erectile function was significantly improved after ADSC treatment via promoting nNOS-positive nerve regeneration and cavernous tissue recovery. The expression of cleaved caspase 3 (an apoptotic marker) reduced after ADSC treatment. Although few EdU-labeled ADSCs were visualized within the penis 4 weeks after administration, plenty of EdU-labeled ADSCs were found around penile dorsal vessels and nerves 1 week after treatment. Furthermore, three neurotrophic factors (NGF, VEGF, and Neurturin) were significantly decreased in Cryo group, and were partially recovered 1 week after ADSC injection. These results suggested that IC injection of ADSC resulted in substantial recoveries of erectile function after CN cryoinjury. The effects may be achieved through the elevated level of neurotrophic factors in penile tissue and subsequent neuroregenerative effects. © 2015 American Society of Andrology and European Academy of Andrology.
