Purpose To compare the effect of photodynamic therapy (PDT) and intravitreal ranibizumab in patients with polypoidal choroidal vasculopathy (PCV). Design Randomized clinical trial. Methods setting: Multicenter. study population: Total of 93 patients with treatment-naïve PCV. intervention: Patients were randomized to 2 arms. Patients in the PDT arm underwent a single session of PDT with verteporfin, and patients in the ranibizumab arm received 3 monthly ranibizumab injections at baseline. Additional treatment was performed as needed in each arm. main outcome measures: Primary outcome measurement was the proportion of patients gaining or losing more than 0.2 logarithm of minimal angle of resolution (logMAR) units from baseline. Mean change of logMAR and central retinal thickness (CRT) were also evaluated. Results In the PDT arm (n = 47), 17.0% achieved visual acuity gain, 55.3% had no change, and 27.7% experienced visual acuity loss. The results were 30.4%, 60.9%, and 8.7%, respectively, in the ranibizumab arm (n = 46), significantly better than the PDT arm (P =.039). In the PDT arm, mean CRT improved (366.8 ± 113.6 μm to 289.1 ± 202.3 μm, P <.001), but logMAR was unchanged (0.57 ± 0.31 to 0.62 ± 0.40). The ranibizumab arm demonstrated improvement in both CRT (418.9 ± 168.6 μm to 311.2 ± 146.9 μm, P <.001) and logMAR (0.48 ± 0.27 to 0.39 ± 0.26, P =.003). Mean change of logMAR was also greater in the ranibizumab arm (P =.011). Conclusion Intravitreal injection of ranibizumab is more effective than PDT for treatment-naïve PCV.

Background: The use of an offset type tensor for total knee arthroplasty that can be set with patellofemoral joint reduction and femoral component placement enables surgeons to assess soft tissues in the physiological postoperative knee condition, showing different kinematic pattern of soft tissues in varus osteoarthritic knees between cruciate-retaining and posterior-stabilized total knee arthroplasty. However, gap kinematics in unicompartmental knee arthroplasty is unclear. Methods: Using a newly developed tensor that is designed to assess soft tissue balance throughout the full range of motion with femoral component placement, we assessed the intra-operative joint gap measurements of unicompartmental knee arthroplasties performed at 0, 10, 30, 45, 60, 90, 120 and 135 of flexion in 20 osteoarthritic patients. In addition, the kinematic pattern of unicompartmental knee arthroplasty was compared with those of cruciate-retaining and posterior-stabilized total knee arthroplasty that were calculated as medial compartment gap from the previous series of this study. Findings: While the joint gap measurements of unicompartmental knee arthroplasties increased from full extension to extension (10 of flexion), these values remained constant throughout the full range of motion. Of note, the gap values of cruciate-retaining total knee arthroplasty were significantly smaller from midrange to deep flexion compared with posterior-stabilized total knee arthroplasty, and furthermore unicompartmental knee arthroplasty showed a significantly smaller gap from extension to midrange flexion compared with cruciate-retaining total knee arthroplasty. Interpretation: Accordingly, we conclude that the intra-operative joint gap kinematic pattern in unicompartmental knee arthroplasty differs from the pattern in total knee arthroplasty. © 2012 Elsevier Ltd.

With the use of an offset type tensor for total knee arthroplasties (TKAs), intraoperative soft tissue balance including the joint component gap and ligament balance was measured in 41 varus-type osteoarthritic patients (19 cruciate-retaining [CR] TKAs and 22 posterior-stabilized [PS] TKAs), and the correlations between the intraoperative values and the postoperative values assessed by stress radiographs at extension and flexion were examined at a minimum 5-year follow-up. In CR TKAs, the postoperative soft tissue balances at both angles were significantly correlated with the intraoperative values. In PS TKAs, the postoperative soft tissue balances at extension, not flexion, were significantly correlated with the intraoperative values. In conclusion, the intraoperative condition of the soft tissue balance reflected the postoperative values especially in CR TKAs even at 5-year midterm follow-ups. © 2012 Elsevier Inc.

We evaluated the outcome of 76 cases of laryngeal cancer primarily treated in our hospital from January 2001 to December 2010. Supraglottic cancer was found in 21 cases (27.6%), glottic cancer in 54 cases (71.1%), and subglottic cancer in 1 case (1.3%). The five-year survival rates were as a whole 82.9%, supuraglottic type 57.2%, glottic type 92.9%, and stage I 100%, stage II 83.6%, stage III 57.1%, stage IV 35.7%, respectively. The five-year organ preservation rates were 82.4% in T1, 61.4% in T2, respectively. Concurrent chemoradiotherapy is important for improvement of survival and organ preservation of laryngeal cancer.

We report two rare cases of extraparotid Warthin's tumor arising from the lateral cervical lymph node. Two 79-year-old men presented with the right lateral neck masses without tenderness and referred pain. Both patients underwent surgical excisional biopy of the tumors for diagnosis. Those masses were easily extirpated without adhesion and there were no communication with the parotid gland. Those specimens were dark-red, slight irregular, slight elastic hard nodular masses. Histopathological examinations of two cases revealed Warthin's tumor. No reccurences were observed after the surgery until now in both cases.

A 67-year-old man with bladder cancer who was treated with transurethral resection of bladder tumour(TUR-Bt)and chemotherapy at the age of 59 years was diagnosed as having urothelial cancer by biopsy 8 years later. Detailed examination revealed the presence of synchronous triple cancer, with hepatocellular cancer and gastric cancer. Subsequently, semi-total gastrectomy, partial hepatectomy(S6), radio frequency ablation(S5, S7), and cholecystectomy were performed. Histologically, the gastric tumor was a moderately differentiated tubular adenocarcinoma, the hepatic tumor was a moderately differentiated hepatocellular carcinoma, the bladder tumor was a transitional cell carcinoma, and the ureteral tumor was an urothelial carcinoma.

The influence of joint distraction force on intraoperative soft tissue balance was evaluated using Offset Repo-Tensor® for 78 knees that underwent primary posterior-stabilized total knee arthroplasty. The joint center gap and varus ligament balance were measured between osteotomized surfaces using 20, 40 and 60. lbs of joint distraction force. These values were significantly increased at extension and flexion as the distraction force increased. Furthermore, lateral compartment stiffness was significantly lower than medial compartment stiffness. Thus, larger joint distraction forces led to larger varus ligament balance and joint center gap, because of the difference in soft tissue stiffness between lateral and medial compartments. These findings indicate the importance of the strength of joint distraction force in the assessment of soft tissue balance, especially when using gap-balancing technique. © 2014 Elsevier Inc.


In the carotid artery plaque diagnosis, it is said that properties evaluation is important. For this inspection, it is general to use electrocardiogram (ECG)-trigger-dark blood (DB)-fat suppression (fs)-turbo spin echo (TSE)-T1 weighted image (T1WI), T2WI, and magnetization prepared rapid gradient echo (MPRAGE) methods though many problems still remain. This time, a comparative study of the carotid artery plaque diagnosis that used the sampling perfection with application optimized contrasts was made. This used different flip angle evolutions method sampling perfection with application optimized contrasts using different flip angle evolutions (SPACE) method that was 3 dimension (3D) imaging procedure that used variable flip angle with the law so far. The subjects were normal volunteers, handmade phantoms changed by T1 and T2 value, and the five patients on five cases who had taken carotid endoarterectomy (CEA) (male, mean 70.6 yr). Findings by the pathologist were obtained for the CEA enforcement patient. There is an excellent result in the contrast ratio by phantom, the signal intensity ratio of a clinical patient, and the comparison with pathological findings. The SPACE method can solve various problems, be evaluated by a properties evaluation of the plaque, a grasp of the range, and an arbitrary section, and be devised as a useful imaging procedure.

A 47-year-old male and a 62-year-old female visited our hospital with the chief complaint of posterior neck pain. In each case, the increased width of the retropharyngeal space without ringed enhancement of the surrounding area and calcification of soft tissue anterior to the Cl and C2 vertebral bodies were identified on CX and the high intensity areas of the retropharyngeal space on T2-weighted images in MRI. The symptoms resolved about one week after conservative treatment with anti-inflammatory and analgesic medications. The clinical courses of both cases were uneventful until now.

A 43-year-old man was transported to our hospital by an ambulance because of failure of tube insertion for general anesthesia by a huge lingual cyst. A 68-year-old man was sufferring from abnormal sensation in the pharynx for two months. In both cases lingual cysts were located in the base of tongue. Each mass was marsupialized with oral approaches under general anesthesia. Histopathological examinations showed mucosal retention cyst in both cases. In the former case, recurrence was observed five months after the first surgery and completely resected by extra-anterior neck approach.
