AIM: To evaluate the efficacy of centralized culture and possible influencing factors. METHODS: From January 2010 to July 2012, 66452 patients with suspected Helicobacter pylori (H. pylori ) infection from 26 hospitals in Zhejiang and Jiangsu Provinces in China underwent gastrointestinal endoscopy. Gastric mucosal biopsies were taken from the antrum for culture. These biopsies were transported under natural environmental temperature to the central laboratory in Hangzhou city and divided into three groups based on their transport time: 5, 24 and 48 h. The culture results were reported after 72 h and the positive culture rates were analyzed by a χ 2 test. An additional 5736 biopsies from H. pylori -positive patients (5646 rapid urease test-positive and 90 14C-urease breath test-positive) were also cultured for quality control in the central laboratory setting. RESULTS: The positive culture rate was 31.66% (21036/66452) for the patient samples and 71.72% (4114/5736) for the H. pylori -positive quality control specimens. In the 5 h transport group, the positive culture rate was 30.99% (3865/12471), and 32.84% (14960/45553) in the 24 h transport group. In contrast, the positive culture rate declined significantly in the 48 h transport group (26.25%; P < 0.001). During transportation, the average natural temperature increased from 4.67 to 29.14 ?, while the positive culture rate declined from 36.67% (1462/3987) to 24.12% (1799/7459). When the temperature exceeded 24 °C., the positive culture rate decreased significantly, especially in the 48 h transport group (23.17%). CONCLUSION: Transportation of specimens within 24 h and below 24 °C. is reasonable and acceptable for centralized culture of multicenter H. pylori samples. © 2015 Baishideng Publishing Group Inc. Background: The resistance of Helicobacter pylori (H. pylori) to antibiotics is increasing worldwide, lowering its efficacy in current eradication therapies. This study evaluated H. pylori resistance to antibiotics in the southeast coastal region of China and suggests appropriate alternatives. Materials and Methods: Seventeen thousand seven hundred and thirty one H. pylori strains were collected from eight areas of two provinces in coastal southeast China from 2010 to 2012. The resistance of these strains to six antibiotics was tested using the agar dilution method. Results: The resistance rates to clarithromycin, metronidazole, levofloxacin, amoxicillin, gentamicin and furazolidone were 21.5, 95.4, 20.6, 0.1, 0.1 and 0.1%, respectively. Double, triple and quadruple antibacterial resistant percentages were 25.5, 7.5 and 0.1%, respectively. A positive association between the resistance to levofloxacin and to clarithromycin was found, but there was a negative correlation in the resistances to levofloxacin and to metronidazole. Conclusions: The prevalence of H. pylori resistance to clarithromycin, metronidazole, levofloxacin and multiple antibiotics in coastal southeast China is high. Choice of therapy should be individualized based on a susceptibility test in this region of the country. © 2013 John Wiley & Sons Ltd.

To recognize the clinical characteristics and outcomes of neonatal diabetes mellitus (NDM), the authors retrospectively reviewed 1 NDM baby in their department and compared their data with 39 NDM cases reported in the available Chinese literature between January 1986 and December 2010. Most of the cases were located near the eastern and southern coasts of China, and clinical manifestation of 72.5% of the cases occurred in 4-week-old infants. Hyperglycemia and glycosuria findings were seen in all the patients and in 47.5% with intrauterine growth retardation. Moreover, 30.0% of the cases had polyuria, 52.5% had dehydration, and 47.5% had ketoacidosis. Cases with hyperglycemia, dehydration, and ketoacidosis recovered mostly. Ten NDM cases had persisted after 1 to 11 years of follow-up, 3 cases maintained normal blood sugar, and 7 cases had poor sugar control. NDM is a rare condition and early management includes fluid and insulin and later management depends on the transient or permanent nature of the condition. © The Author(s) 2012.

Background: Rapidly involuting congenital hemangioma is a congenital soft tissue tumor, which is difficult to excise completely and rather prone to recur. This atypical tumor combined with capillary-lymphatic-venous malformation was not reported in the literature. Case presentation: We report an atypical case of a 16-year-old teenager who was born with a mass in his right upper limb. Since there is a recurrence after excision for several times and had a serious impact on daily life, we chose amputation. After the surgery, the patient gained a functional recovery. Two years after the surgery, he had no tumor recurrence. Conclusions: For this rare tumor with repeated recurrences and significant impact on daily life, we suggest performing amputation at the early stage. © 2016 The Author(s).

Ficolin-mediated activation of the lectin pathway of complement contributes to the complement-independent inflammatory processes of traumatic brain injury. Lower serum ficolin-3 levels have been demonstrated to be highly associated with unfavorable outcome after ischemic stroke. This prospective observatory study was designed to investigate the relationships between serum ficolin-3 levels and injury severity and clinical outcomes after severe traumatic brain injury.Serum ficolin-3 levels of 128 patients and 128 healthy controls were measured by sandwich immunoassays. An unfavorable outcome was defined as Glasgow Outcome Scale score of 1-3. Study endpoints included mortality at 1week and 6months and unfavorable outcome at 6months after head trauma. Injury severity was assessed by Glasgow Coma Scale score. Multivariate logistic models were structured to evaluate the relationships between serum ficolin-3 levels and study endpoints and injury severity.Compared with the healthy controls, serum ficolin-3 levels on admission were statistically decreased in patients with severe traumatic brain injury. Serum ficolin-3 levels were independently correlated with Glasgow Coma Scale scores. Ficolin-3 was also identified as an independent prognostic predictor for 1-week mortality, 6-month mortality, and 6-month unfavorable outcome. Under receiver operating characteristics curves, ficolin-3 has similar prognostic predictive values for all study endpoints compared with Glasgow Coma Scale scores. It was proposed that lower serum ficolin-3 levels, correlated with injury severity, had the potential to be the useful, complementary tool to predict short- or long-term clinical outcomes after severe traumatic brain injury.

To evaluate the clinical efficacy of artificial joint replacement in treatment of patients with metastatic tumors involving the proximal femur. A total of 7 patients with metastatic tumors involving the proximal femur treated with artificial joint replacement were selected from Sanmen People's Hospital, including metastatic breast carcinoma in 2 patients, lung carcinoma in 2 patients, prostate carcinoma, renal carcinoma and colorectal carcinoma in 1 patient each. Five of the 7 patients suffered pathologic fracture during their course of disease. All patients were treated with surgical removal of tumors and artificial joint replacement, including 5 patients with hemiarthroplasty and 2 with total hip arthroplasty. All the 7 patients received complete follow-up for a mean of 21 months. There were no such postoperative complications as local infection, prosthesis dislocation, prosthesis loosing or subluxation. The average survival time was 26 months (5-67 months). According to the Ennecking functional scoring system, the patients were rated as excellent in 4 and good in 3, and the excellent and good rate was 100.0%. Conclusion Artificial joint replacement for patients with metastatic tumors involving the proximal tumor could lead to significant improvement in pain relief, affected limb functional recovery and quality of life.

Background: Preoperative diagnosis of gastric stromal tumor (GST) depends mainly on endoscopy and imaging examination. For improving the preoperative diagnostic level of GST, a comparative study on the coincidence of CT and postoperative pathologic diagnosis should be performed. Aims: To determine the diagnostic value of CT in GST. Methods: Twenty-four cases of GST diagnosed in Sanmen People's Hospital, Zhejiang Province from Jan. 2002 to Jun. 2008 were enrolled. The CT and histopathologic features were analyzed retrospectively. Results: The GST in all 24 cases were solitary, of which the diameter ranged from 1.3 cm to 12.5 cm. Thirteen GST had their diameter less than 5 cm, while 11 were ≥5 cm. Four GST were located intraluminally, 14 were extraluminal, and 6 were transmural. Eight GST showed homogenous density on plain scanning, whereas 16 were heterogeneous with gas in 4. Malignancy was diagnosed in 9 cases by CT, malignant suspected in 6, and benign in 9. In pathologic diagnosis, malignancy was found in 10 cases, borderline in 5, and benign in 9. Only in one case the diagnosis by CT and pathology was different. Conclusions: The CT features of GST are characteristic, which has important value in the preoperative diagnosis of GST and in the differentiation between malignant and benign lesions.

Hepatocellular carcinoma (HCC) is one of the most frequent primary malignancies of the liver and is resistant to anticancer drugs. Hypoxia is a master cause of tumor resistance to chemotherapy. Hypoxia-inducible factor-one alpha (HIF-1α) plays a key role in the adaptive responses to hypoxic environments. HIF-1α is constitutively up-regulated in several tumor types might thus be implicated in tumor therapy resistance. We hypothesized that disruption of HIF-1α pathway could reverse the hypoxia-induced resistance to chemotherapy. In this report, we prepared DOTAP (a liposome formulation of a mono-cationic lipid N-1-(2,3-Dioleoyloxy)]- N,N,N-trimethylammonium propane methylsulfate in sterile water) cationic liposomes containing an antisense oligonucleotide (AsODN) against HIF-1α. Gene transfer of antisense HIF-1α was effective in suppressing tumor growth, angiogenesis, and cell proliferation, and inducing cell apoptosis. Our results suggested that antisense HIF-1α therapy could be a therapeutic strategy for treating HCC. © 2015, E-Century Publishing Corporation. All rights reserved.
