Objective To investigate the epidemiological characteristics of congenital heart diseases (CHD) of young Tibetans aged from 4 to 18 years resided above 4000m (mean 4200m) altitude in Qinghai province. Methods A total of 25185 young Tibetan resided in Yushu or Guoluo (above 4000m altitude, mean 4200m) were surveyed with the following 3 steps: prescreen, secondary screen, and diagnosis with color Doppler echocardiography. The incidence, distribution of disease, and difference between genders and age periods of CHD were analyzed. The incidence of CHD in Yushu reported in 1980s was compared with the result of present investigation. Results Two hundred and twenty cases of CHD were diagnosed in the two regions, and the total incidence was 8.74‰. There was no significant difference in incidence of CHD between Guoluo (9.79‰, 63/6438) and Yushu (8.37‰, 157/18 747), but the incidence in Yushu of the present study was lower than that reported in 1980s. The total standardized incidence of CHD in females (20.73‰) was significantly higher than that in males (13.83‰, P<0.01). In Yushu, with the increase in age, the incidence of CHD showed an increasing tendency (P<0.01), and it was significantly higher in those aged 16 to 18 years (15.74‰) than those in aged 4 to 7 years (5.39‰, P<0.01) and 8 to 12 years (7.94‰, P<0.05). Increasing tendency of CHD incidence was also found in Guoluo with an increase in age (P<0.05), and the incidence in those aged 13 to 15 years (14.08‰) was higher than those aged 4 to 7 years (3.65‰, P<0.05). The predominant type of CHD in Yushu or Guoluo was patent ductus arteriosus (PDA, 46.36%), followed by atrial septal defect (ASD, 35.45%) and ventricular septal defect (VSD, 13.64%). Conclusions The incidence of CHD in young Tibetan aged 4 to 18 years resided above 4000m altitude was found to be significantly decreased compared with that reported in 1980s, but it showed an increasing tendency with the increase in age. PDA was the predominant type of CHD in the two regions.
