The paper presents clinical supervision of child’s post-traumatic pancreatic cyst. The described case demonstrates efficiency of comprehensive approach to evoked cyst treatment which comprises acute pancreatitis therapy with dynamic assessment of the results and subsequent surgical measure. The authors demonstrated capabilities of ultrasound investigation in this pathology diagnostics and conservative treatment complexity. The supervision is of interest because of unconventional approach to the operation: the patient received cyst laparoscopic drainage and sclerotherapy. The review presents the comparative analysis of current surgical techniques to treat atherosclerotic carotid lesions: carotid stenting and carotid endarterectomy (classical and everting), and describes the advantages and disadvantages of these methods introducing indications and contraindications. Particular emphasis is given to the technique and time to complete carotid endarterectomy. The paper concerns the benefits of a patch over a primary suture, as well as presents the requirements for a patch used in carotid endarterectomy. The authors studied all current materials used for patches, and gave merits and demerits of each material described. The review assesses the feasibility and safety of the more invasive surgical approach for brain revascularization in an acute period of ischemic stroke in strict compliance with surgical indications. The authors have concluded that carotid endarterectomy has a positive effect on cognitive functions in patients in the form of neurological and neuropsychological status improvement. There has been also proved that all above mentioned techniques have no advantages, moreover, they can be mutually complementary and, actually, used in chronic cerebral ischemia treatment. © 2015, Nizhny Novgorod State Medical Academy. The aim of the investigation was to work out a concept of modern arthrodesis of the ankle joint in posttraumatic crusarthrosis, grade III–IV, using innovative fixing implants. Materials and Methods. The results of treating 53 patients covering the period from 1 to 15 years after 56 operations of arthrodesis, performed for posttraumatic crusarthrosis, grade III–IV (according to Kellgren and Lawrence classification), have been analyzes. The conclusions were based on the data of radiographic, clinical examination and the scoring according to the international rating scale AOFAS. Arthrodesis was performed using state-of-the-art fixing implants: with the help of the screws, retrograde intramedullary HAN rod, Ilizarov apparatus for external fixation, wires. Results. In the majority of patients (41%) the main cause of developing posttraumatic crusarthrosis of grade III–IV was unrepaired intraarticular dislocation of the ankle fragments and a large fragment (more than 1/3) of the posterior tibia margin. The best results, i.e. fewer ligamentous ankylosis (32%) and the greatest number of good (39%) and satisfied (54%) results according to AOFAS scale were obtained when crossed screws were used in arthrodesis. It was estimated, that the assessment of clinical and functional signs according to the international scale AOFAS does not reflects the most vital spectrum of life activity indices of patients, therefore, it has been suggested to use additionally individualized indices characterizing the quality of life of patients of various professions, levels of every-day activity and psychological motivation to adequate rehabilitation. Unsatisfied radiographic findings did not always corresponded to the poor clinical results. In 56% of patients with ligamentous ankylosis the result according to AOFAS was good or satisfactory (slight pains in prolonged walking, moving without additional supporting means, absence of evident limping). Conclusion. The worked out concept of modern arthrodesis is as follows: 1) the choice of ankle arthrodesis method and the type of a fixing implant must depend not only on the type of deformation, but on the patient’s age, gender traits, diseases contributing to the development of unfavorable treatment results; 2) the best clinical and functional results are connected with the method of fixation providing primary stability, and with regenerative and adaptive capabilities of patients in diverse age groups; 3) the most optimal state-of-the-art implants for arthrodesis are crossed screws for fixation, providing the greatest strength and good primary compression, if there is no marked disorder of blood supply of the articular bones and radical removal of the cartilaginous tissue. © 2016, Nizhny Novgorod State Medical Academy. The review covers the current state of the problem of operative treatment of malleolar fractures and the associated consequences. Various methods used in osteosynthesis of the malleolus were analyzed. The main criteria of the favorable arrangement of the fractured bone fragments for malleolus reposition are identified. Methods for recovery of the ligamentous apparatus of the ankle joint are described, that have been neglected by many surgeons, though in 50% of cases it causes development of chronic instability with further osteoarthrosis. The reason for large number of poor outcomes of malleolar fracture treatment is defined, which occur despite the anatomically precise reposition, recovery of the ligamentous apparatus and good postoperative care of patients. All this is mainly due to presence of post-traumatic articular cartilage defects that further induce development of osteoarthrosis. The main techniques used for arthroscopic treatment of this pathology are described, including that with the autologous chondrocyte implantation. Attention is given to the subject of primary and revision replacement of the ankle joint, the long term results in comparison with replacement of knee and hip joints are shown. The modern and most efficient techniques of the ankle joint arthrodesis are presented, including arthrodesis with a retrograde intramedullary rod and arthroscopic arthrodesis. The aim of the investigation was to assess the capabilities of spinal cord stimulation, and compare it with conventional nonsurgical treatment in failed back syndrome. Materials and Methods. The study involved the patients with at least one anatomically successful lumber disc hernia excision, posttraumatic spinal deformity in their past history, the patients keeping suffering from neuropathic pain. Group 1 patients (n=45) in addition to traditional therapy underwent spinal cord stimulation. Group 2 control patients (n=35) were given medicines, therapeutic blocks, physiotherapy, and psychotherapy. Results. Epidural spinal cord stimulation in chronic pain syndrome treatment enables to improve significantly the patients, condition: it lowers pain level, increases the functionality and improves the quality of life. Conclusion. Being minimally invasive and highly safe, neuromodulation enables to achieve good treatment results in failed back syndrome. © 2015, Nizhny Novgorod State Medical Academy. The influence of electromagnetic radiation of extremely high frequencies in the noise mode on the level of the activity of proand antioxidant systems in a model of skin-flap ischemia was studied in vivo. Rats of the Wistar line were used in this work. The cutaneous reflex projection field of the animals was exposed once a day for 7 days in the post-operative period to electromagnetic radiation of an extremely high frequency in the range from 53.57 to 78.33 GHz with a dose of 1.2 mJ in the noise mode. The dynamics of proand antioxidant protection were compared to those of intact animals that were not irradiated. Using data on induced biochemiluminescence and estimation of malondialdehyde activity it was shown that irradiation in a course mode contributed to a decrease in the intensity of lipid peroxidation. In parallel we observed an increase in the total antioxidant reserves of blood and the activity of bioradical protection enzymes. Our findings suggest that electromagnetic radiation at extremely high frequencies in a noise mode has a regulatory impact on the state of proand antioxidant systems of the body; this can be used for the correction of postoperative ischemic disorders and the restoration of disturbed homeostasis. © 2016, Pleiades Publishing, Inc.

The development of new rehabilitation approaches to neurological and traumatological patients requires understanding of normal and pathological movement patterns. Biomechanical analysis of video images is the most accurate method of investigation and quantitative assessment of human normal and pathological locomotion. The review of currently available methods and systems of optical human motion analysis used in clinical practice is presented here. Short historical background is provided. Locomotion kinematics analysis using passive marker based systems is reviewed with special attention to the gait analysis. Clinical application of optical motion capture and analysis systems in the diagnosis of locomotion impairment, in Parkinson's disease with movement control disorders, stroke sequelae, monitoring of motor function rehabilitation in patients with infantile cerebral paralysis, limb joint endo- and exoprosthetics and some other disorders is described.. © 2015, Nizhny Novgorod State Medical Academy. This paper presents the results of the study of oxidative metabolism of the blood of intact animals subjected to prolonged exposure to nitric oxide at concentrations of 20, 50, and 100 ppm. The experiment was carried out with Wistar rats. NO inhalation was performed for 30 days. The state of blood oxidative metabolism was evaluated after inhalation and a 30-day-long recovery period after discontinuation of NO oxidative stress. The intensity of lipid peroxidation was studied in plasma and erythrocytes by induced biochemiluminescence and the measurement of the level of malondialdehyde. The activity of superoxide dismutase was determined in the hemolysate of erythrocytes. It was established that the optimal dose of inhaled NO is 20 ppm: a maximum increase in the total antioxidant activity after 30 days and normalization of lipid peroxidation in the blood after the completion of the recovery period were observed at this concentration. High concentrations of nitric oxide (50 and 100 ppm) initiated lipid peroxidation in erythrocytes and plasma after discontinuation of NO oxidative stress (after the completion of the recovery period) thus enhancing catalytic properties of superoxide dismutase. © 2016, Pleiades Publishing, Inc.

The paper describes successful treatment case of medicamentally-resitant chronic neuropathic pain syndrome of 63 year old patient with cerebro-spinal form of disseminated sclerosis. Metabolic processes were investigated in plasma and erythrocytes of Wistar rats exposed to daily 10-min sessions of NO inhalation for 30 days. These included determination of glucose and lactate, catalase activity, and activities of aldehyde dehydrogenase (ALDH), lactate dehydrogenase (LDH), and catalase. NO inhalation in a concentration of 20 ppm, 50 ppm and 100 ppm caused an increase in glucose and lactate. Inhalation of 100 ppm NO also increased catalase activity. Inhalation of all NO concentrations resulted in a decrease of ALDH activity, while the decrease in LDH activity was observed at NO concentrations of 50-100 ppm.
