The combination of COPD and osteoarthritis (OA) is quite common in clinical practice, reducing opportunities for physical rehabilitation of the patient. The systemic inflammation in each of comorbid diseases defines the status of the immune system, in particular its cellular link. In order to study cell immunity of patients with COPD concomitant with OA we examined 54 patients with exacerbation of COPD of a clinical group B - GOLD II, in the middle ages (54,4 ± 3,1) years and period of the disease (9,6 ± 1,1) years. Of these, patients with acute exacerbation of COPD, comorbid with OA in the phase of unstable remission were 33 people - the main group, with COPD without OA comorbidity - 21 patients (comparison group). The control group included 25 healthy persons of the same sex and age. In all surveyed, in addition to routine clinical and instrumental parameters, T-, B-lymphocytes, T-helpers/inductors (Th), T suppressor/killers in the test with monoclonal antibodies classes of CD3+, CD4+, CD8+ and CD22+ were determined. In patients with acute exacerbation of COPD, comorbid with OA, the decreased number of T-lymphocytes with phenotype CD3+ in absolute terms by 1.7 times (p<0.05) in the relative - in 1,3 times (p<0.05), fewer T-lymphocyte helpers in 1,7 and 1,4 times (p<0.05) were noted, respectively, while maintaining constant rates of T-lymphocytes with the phenotype CD8+. The indicators of cellular immunity in patients with COPD without comorbidity were also decreased but less significantly. Between the levels of CD3+-, CD4+-cells and FEV1 of the patients of the main group, as well as between CD3+-cells and FEV1 of the patients in the comparison group positive correlation dependences were revealed, and negative correlation was revealed between the amount of CD3+-lymphocytes and the level of pain in patients of the main group according to pain scale, indicating a negative effect of OA on clinical presentation of COPD.


After epidemiological, clinical, psychopathological and psychodiagnostic study of 249 patients with non-psychotic mental stress disorders, there were determined gender-specific features of emergence and further clinical course of diseases. Most frequently women suff ered from PTSD and depressive disorders, men suff ered from somatoform disorders.

Exudative or proliferative processes accompanied by joint destruction may predominate in the synovial membrane (SM) in different stages of rheumatoid arthritis (RA). The features of SM changes should be considered in the early diagnosis of RA and in the development of combination treatment for this disease. Objective: To study arthroscopic SM changes in patients with different RA durations and different blood anti-cyclic citrullinated peptide (anti-CCP) antibody levels. Subjects and methods. 37 patients with RA underwent arthroscopy with an arthroscope of a 2.4-mm diameter and 30° angle (Karl Storz GmbH, Germany). RA duration was <2 years in 17 patients and ranged from 2 to 8 years in 20; the blood level of anti-CCP antibodies did not exceed 60 IU/ml in 15 patients and it was higher in 22. SM changes were assessed by a semiquantitative method. Results and discussion. Arthroscopy revealed a preponderance of SM hyperemia with an increased vascular pattern (p<0.01) and fibrin deposits (p<0.05) in patients with a RA duration of < 2 years and that of villous hyperplasia with the formation of club-shaped villi (p<0.05) in those with a RA duration of >2 years. The increase in anti-CCP by >60 U/ml was associated with a predominance of inflammatory hyperplasia (p < 0.01), SM hyperemia with a pronounced vascular pattern (p<0.05), as well as with the presence of pannus (pp<0.01).

The aim of the investigation was to assess the correlation between the main ultrasound indices of joints and clinical, laboratory and instrumental indices in patients with rheumatoid arthritis (RA). Materials and methods. A total of 114 patients with RA underwent complex clinical, laboratory and instrumental examination, including clinical assessment of the patients’ health, according to RA activity index DAS28. Concentrations of C-reactive protein (CRP), TNF-α, anti-cyclic citrullinated peptide antibodies (ACPA), angiogenesis marker VEGF and proliferation factor FGF were measured using ELISA technique. Ultrasound examination of knee joints was carried out using ESAOTE MyLAB40 device (The Netherlands), the knee joint arthroscopy was performed using an arthroscope (Karl Storz, Germany) with a diameter of 2.4 mm and an angle of 30°. The synovial membrane samples were fixed in 10% buffered formalin solution, stained with hematoxylin and eosin and studied using Axiostar microscope (Carl Zeiss, Germany). Results. There were established statistically significant correlations between the deterioration of ultrasound indices of synovitis, synovial membrane vascularization and the increase in the markers of immune-inflammatory RA activity and VEGF marker in the blood, which indicated decline in clinical assessment of the patients’ health. The increase in ultrasound indices of synovial membrane thickness, pannus, osteochondral erosions closely correlated with the increase in RA duration, high blood levels of CRP, ACPA and FGF, which also spoke of deterioration of the clinical indices of the disease and radiographic progression of joint destruction. Conclusion. The intensity of immune-inflammatory, proliferative and destructive processes and the synovial membrane angiogenesis can be assessed by ultrasound examination of joints in RA patients. The data obtained provide the possibility of recommending the application of the non-invasive technique of synovial membrane ultrasound for diagnosis and monitoring treatment efficacy in RA patients instead of expensive invasive techniques. © 2017, Nizhny Novgorod State Medical Academy. The Alzheimer’s disease (AD) is acknowledged as the most common type of dementia in aging adults. It is characterized by the formation of intracellular neurofibrillary tangles and extracellular amyloid plaques. The latter insoluble deposits mainly consist of β-amyloid peptides (Aβ), which are the derivatives of the amyloid precursor protein (APP). The formation of neurotoxic Aβ-peptides involves the cleavage of APP with beta-secretase enzyme (beta-site APP cleaving enzyme 1, BACE1) so the potential of BACE1 inhibitors as therapeutic agents for AD is now drawing much attention. The patent application WO2016023927 reports the preparation of new 1,2,4-thiadiazine inhibitors of BACE1 activity and their use as therapeutically active substances. Some of the new compounds are claimed to be good inhibitors with the IC50 values in the 0.000292–0.134165 μM range. Several pharmaceutical preparations based on these compounds are proposed for possible treatment and prevention of AD. Expert opinion: In light of the novelty from the chemical point of view and improved biological activity, the reported 2,2,2-trifluoroethylthiadiazines could be considered as promising BACE1 inhibitors. However, the available data are insufficient to make a recommendation if these compounds can be considered as drug candidates. Further studies with a larger number of compounds are required. The compounds described in the patent have to be characterized more thoroughly from the chemical viewpoint (e.g., by means of IR, 1H and 13C NMR spectroscopy, X-ray crystallography), especially as regards stereochemical details. © 2016 Informa UK Limited, trading as Taylor & Francis Group.

Chronic hepatitis C patients often fail to respond to interferon-based therapies. This phase III study aimed at confirming the efficacy and safety of glycyrrhizin in interferon + ribavirin-based therapy non-responders. A randomised, double-blind, placebo-controlled, comparison of glycyrrhizin, administered intravenously 5×/or 3×/week, and 5×/week placebo for 12 weeks to 379 patients, was followed by a randomised, open comparison of glycyrrhizin i.v. 5×/versus 3×/week for 40 weeks. Primary endpoints were: (1) the proportion of patients with â¥50% ALT (alanine aminotransferase) reduction after 12 weeks double-blind phase, and (2) the proportion of patients with improvement of necro-inflammation after 52 weeks as compared with baseline. The proportion of patients with ALT reduction â¥50% after 12 weeks was significantly higher with 5×/week glycyrrhizin (28.7%, P < 0.0001) and 3×/week glycyrrhizin (29.0%, P < 0.0001) compared with placebo (7.0%). The proportion of patients with improvement in necro-inflammation after 52 weeks was 44.9% with 5×/week and 46.0% with 3×/week, respectively. Glycyrrhizin exhibited a significantly higher ALT reduction compared to placebo after 12 weeks of therapy and an improvement of necro-inflammation and fibrosis after 52-weeks treatment. Generally, glycyrrhizin treatment was well tolerated. © 2012 Blackwell Publishing Ltd.

Aim. To study plasma lipid profile, systemic inflammation and endotelial function in patients with acute period of MI with Q-wave and liver steatosis (LS) or non-alcoholic steatohepatits (NASH). Material and methods. Totally 48 patients with acute MI of the left ventricle were studied (1st group), with average age 66,33±1,66; 53 — with acute MI and LS (2nd group) — average age 63,74±1,87, and 7 — with acute MI and NASH (3rd group) — average age 58,86±4,47. In all groups men dominated. We measured the levels of liver enzymes, bilirubin, total cholesterol, LDP cholesterol, VLDL cholesterol, triglycerides, atrogenity coefficient, cholesterol-accepting properties of HDL, level of nitrite- and nitrate-anions in serum, hsC-reactive protein. Results. The disordered lipid profile in patients of both groups shows the increased levels of total cholesterol, LDL (p<0,001) and VLDL (p<0,01), atherogenity coefficient (p<0,001) and lowering of HDL and their cholesterol-accepting properties (p<0,001). In patients of 2nd and 3rd groups there is an increase of nitrites (p<0,001) and nitrates (p<0,001). Endothelial dysfunction was the worse in acute MI with NASH. In patients with combined pathology there is increase of C-reactive protein (p<0,001). The parameter of systemic inflammation is higher in patients with acute MI with NASH. Conclusion. In patients with combined MI and LS or NASH there is an increase of atherogenity and decrease of anti-atherogenity in lipid profile, increase of stable NO metabolites with maximally prominent endothelial dysfunction in patients with NASH, due to aseptic inflammation. In the article the sensitivity of new vectorcardiographic research method of patients on the cardiodiagnostic complex МТМ-SКМ in the process of thrombolytic therapy is proved by clinical example. Clinical case. In a patient with acute myocardial infarction of anterior wall of the left ventricle vectorcardiogram results give the opportunity to confirm a significant diminution of the area of myocardial damage with concomitant deterioration of conductivity and preserving of ischemic events due to reduction of electromotive force of the heart in the anterior wall of the left ventricle. In addition, the dynamic decrease of the electrical activity of the atria with a significant slowing of the pulse and repolarization disorder may indicate involvement of atriums in the pathological process with a considerable blood flow disorder in them. Conclusion.This method allows to make a quality and quantitative evaluation of the efficiency of the reperfusion therapy, as well as to obtain the information about the electromotive force of the heart both in the necrobiosis zone and out of myocardium affection zone. © 2015, Silicea-Poligraf All rights reserved.

Aim. To reveal vectorcardiographic (VCG) signs of an acute widespread myocardial infarction (MI) with Q, of the anterior wall of the left ventricle (LV), comorbid with hepatic steatosis and non-alcoholic steatohepatits, and to estimate the specificity and sensitivity of the method in diagnostics of comorbid pathology. Material and methods. Totally, 14 patients studied with primary widespread MI of the anterior wall of LV (1st group), mean age - 69±3 y.; 43 patients with comorbid course of primary MI of anterior wall and hepatic steatosis or non-alcoholic steatohepatitis (2nd group) - mean age 66±2 y. In both groups women predominated. Instrumental study included electrocardiography, VCG, echocardiography and ultrasound liver examination. VCG performed on a modern cardiodiagnostic multifunctional equipment MTM-SKM. For estimation of sensitivity and specificity of VCG the four-pole net was applied. Results. In comorbidity patients via VCG there was shifting of QRS loop down-, right-and backwards with a decrease of collected square 2,1 times (p<0,001); appearance of local ventricular block; disturbance of repolarization appearing as ST-damage vector, directed right-up-frontwards (51,2%) and left-frontwards (48,8%), changes of T-loops location, angular QRS-T loops direction in VA1,2,4,5 (p<0,05-0,001), increase of T-loops square 2,9-3,0 times in VA2,3,5 (p<0,01), of their cross and condensation of timing marks by their trace in all projections (p<0,001). Rarefaction of time points in the area of baseline shift of the loop in VA1,4,5 (p<0,05-0,001), increase of the loops QRS square in VA5 (p<0,01) and loops P in VA1-3 2,4-3,2 times (p<0,01) witness hemodynamic overload of basal areas of both ventricles, higher areas of front and lateral walls of LV, and also atriums. VCG let to diagnose non-transmural involvement of myocardium in all patients with comorbid pathology. In 2nd group patients, comparing to the 1st group, there is rarefaction of time marks in the area of baseline shifts of QRS loops in anterior projection (p<0,01), that represents overload in upper areas of LV front wall. In echocardiography it is found that left chambers increase (p<0,001), there is diastolic dysfunction (relaxation disorder of LV - 40%), pseudonormal type - 32%, restrictive type - 28%) with systolic function impairment (p<0,001). Sensitivity of vectrocardiography was 98,5%, and specificity - 95,2%. Conclusion. Application of enhanced cardiodiagnostic complex MTM-SKM and vector analysis of electromotive force made it, with sensitivity 98,5% and specificity 95,2%, to reveal and clarify the depth and spread of myocardial damage, additionally to electro-and echocardiographic methods, and to obtain information on the heart condition outside necrosis zone. © Russian Journal of Cardiology.

A method of Pareto-optimal synthesis is worked out for axisymmetric magnetic systems containing ferromagnetic elements with nonlinear magnetic properties. The method is based on the simultaneous application of the swarm intelligence paradigm and the evolution optimization with the help of genetic algorithms.
