During urologic work-up in a 69-year-old man, a cystic bowel wall thickening was incidentally detected and turned out to be a mesenteric lymphangioma. Cystic lymphangiomas are rare, benign malformations of the lymphatic system which occur predominantly in childhood. However, they should be considered in the differential diagnosis of abdominal cystic lesions or acute abdomen at any age. Preoperative management consists of radiologic studies (ultrasound, CT, MR). Even when lymphangiomas are asymptomatic, radical surgical excision should be performed because of potential complications such as bleeding, rupture, volvulus, and infection. © 2016 Hogrefe.
