Objective. To establish correlations between parameters of the neurological status of patients in the acute period and quality-oflife in the late recovery period of atherotrombotic stroke in carotid and vertebrobasilar areas. Material and methods. One hundred and sixty-two patients, including 124 with carotid stroke and 28 with vertebrobasilar stroke, were examined. Quantitative parameters of the neurologic status were estimated during the hospitalization of patients. Quality-of-life assessed with the «SF-36 Health Status Survey» with the calculation of two parameters (mental (MH) and physical (PH) wellbeing) was evaluated 10-14 months after stroke. Results and conclusion. Parameters of the neurologic status had an effect only on the PH domain of qualityof- life while MH was independent of the indicators studied. The initial NIHSS index and Rivermead Mobility Index had the greatest impact on PH in patients with carotid stroke. In patients with vertebrobasilar stroke, the strong correlation of PH with the age of patients and an index of independence, measured with the Rankin scale on the 14th day of hospitalization, was found. © 2017, Media Sphera. The main parameters of free radical oxidation and antioxidant defense in the blood plasma, erythrocytes, and homogenates of skeletal muscles, heart, liver, lungs, and kidneys were studied in adult outbred albino male rats with different degree of adaptation to moderate exposure to swimming. The rats were trained to swim regularly over 1 month. Changes in oxidative balance varied in organs and tissues and depended on the level of training. Malonic dialdehyde content in the erythrocytes after swimming increased by 13.8% in non-trained animals, but decreased by 19.2% in trained rats. Parameters of blood plasma reflect the general oxidative balance of organs and tissues.

Objective. To assess quality of life of patients with first-ever atherothrombotic stroke in the carotid artery territory 2 years after the disease onset. Material and methods. Authors examined 119 patients. Quality of life was measured with the ≪SF-36 Health Status Survey≫ using two parameters: level of mental (MH) and physical (PH) health. A control group consisted of 20 sex- and agematched healthy persons without neurological diseases. Results. After two years, 67% of the patients assessed their health condition as satisfactory. In the patients, a decrease in PH was greater compared to MH than in controls. Conclusion. In poststroke period, patients for a long time had lower quality of life indices with marked physical impairment. Elimination of or reduction in the focal neurological deficit that led to the decrease in the level of physical wellbeing may be considered as a significant rehabilitation reserve. © 2015, Media Sphera. Cardiac repolarization is prolonged in diabetes mellitus (DM), however the distribution of repolarization durations in diabetic hearts is unknown. We estimated the ventricular repolarization pattern and its relation to the ECG phenomena in diabetic mice. Potential mapping was performed on the anterior ventricular surface in healthy (n=18) and alloxan-induced diabetic (n=12) mice with the 64-electrode array. Activation times, end of repolarization times, and activation-recovery intervals (ARIs) were recorded along with limb lead ECGs. ARIs were shorter in the left as compared to right ventricular leads (P<0.05). The global dispersion of repolarization, interventricular and apicobasal repolarization gradients were greater in DM than in healthy animals (P<0.03). The increased dispersion of repolarization and apicobasal repolarization gradient in DM correlated with the prolonged QTc and Tpeak-Tend intervals, respectively. The increased ventricular repolarization heterogeneity corresponded to the electrocardiographic markers was demonstrated in DM. © 2012 Institute of Physiology v.v.i.

The aim of the investigation is to study the possibilities of using modern combined cryodestruction technique to treat ingrown nail in polyclinic surgeries. Materials and methods. There have been analyzed the results of complex cryosurgical treatment of ingrown nail in 17 patients with various degree of pathological process. 17 patients of the control group underwent traditional surgical treatment. Conclusion. Cryosurgical method of treating patients with ingrown nail is least traumatic, cost-effective. It contributes to rapid rehabilitation, improvement of patient's life quality, vocational rehabilitation, and has a good cosmetic effect.

The aim of the investigation was to assess the efficacy of using air-gas mixture as a substitute for vitreous body in microinvasive surgical management of primary pseudophakic retinal detachment (PRD) and its recurrence after extrascleral intervention performed by 25G endovitreal surgery technique. Materials and Methods. Two groups of patients, which underwent vitrectomy with tamponade of the vitreal cavity by silicone oil or gas, were formed: group 1 (n=24) with primary unoperated PRD and group 2 (n=10) with recurrent PDR after extrascleral surgery. Both groups were further divided into subgroups: A — patients with vitrectomy and introduction of silicone oil; B — patients with vitrectomy and air-gas mixture. Results. A comparative analysis of PRD surgical treatment data, carried out in our investigation, showed that in case of primary PRD treatment higher anatomical and functional results were achieved, when vitrectomy was made with application of silicone oil, which performed more controlled and prolonged tamponade of the vitreal cavity than in vitrectomy with replacement of the vitreous body by air-gas mixture. In case of PRD recurrence after extrascleral surgery greater efficacy in comparison with the application of the silicone oil was revealed in vitrectomy and air-gas tamponade of the vitreal cavity if there were no coarse retinal fibrosis and large extension of redetached retina. Conclusion. Only differentiated approach to the choice of temporal substitute of vitreous body after vitrectomy in patients with pseudophakic retinal detachment will allow the surgeons to achieve maximally feasible anatomical and functional results. © 2016, Nizhny Novgorod State Medical Academy. Using immunohistochemical and morphometric methods, the quantitative composition and structural localization of immunocompetent cells were studied in human spleen (n=20, autopsy material). The detailed morpho-functional characteristics of splenic white and red pulp are presented. Cell subpopulations of T (CD3+, CD4+, CD8+) and B lymphocytes (CD20+), natural killer cells (CD57+), tissue macrophages (CD68+) and dendritic cells (CD35+, S100+) were studied. An analysis of the correlations of immunocompetent cells in different regions of white and red pulp may become the basis for optimization of histopathological diagnosis definition in clinical practice


Background. Bronchial asthma (BA) is a serious problem. It was found that the frequency of arterial hypertension (AH) detection in patients with asthma is about 30 %. At patients practically all types of violations of a warm rhythm meet a bronkhoobstruktivny syndrome. Emergence of rather new preparations - If-blockers, opens new opportunities in correction of heart rate (HR) and a cardioprotection at a combination of AH and BA. Aim. To assess therapy diltiazem-retard and ivabradine on structurally functional changes of a cardiac muscle and HR at patients with AH in combination with BA. Material and methods. Patients (n=91) with BA with AH 1, 2. All patients were outpatients and taken therapy on BA with inhaled corticosteroids. The patients used a p2-Agonists. All patients were performed the echocardioscopy on Acuson 128XP/ 10c (USA). Holter monitor ECC (HM-EKG) was found myocardium ischemia - 18 patients, research was continued by 73 persons. The patients were divided into 2 groups. In the first group made n = 37, age 53 ±7,64, received diltiazem-retard. The second group included n = 36, age 51,2±7,13 years - ivabradine. Duration of observation is 12 weeks. Results and conclusion. Therapy ivabradine and diltiazem at patients BA according to HM-EKG reliable decrease in frequency of registration of ventricular extrasistoliya in days and heart rate (HR) (p<0,05), the quantity of supraventricular extrasistoliya per day doubtfully decreased. Diltiazem wasn't more effective concerning decrease in HR, ventricular and supraventricular extrasistoliya, when comparing with ivabradine (p>0,05). The result of 12 week therapy ivabradine at patients with a combination of AH and BA more significant was observed in comparison with diltiazem, regression a hypertrophy of the left ventricle, improvement of diastolic function and almost full regression of warm ektopiya. The tendency was to decrease and stabilization arterial pressure and decrease in HR to purpose level.

Hypothesis: local and global activation-to-repolarization coupling differ from each other. Experiments were done in 12 dogs and 7 frogs. Activation times (ATs), end of repolarization times (RTs), and activation-recovery intervals (ARIs) were measured in 24 or 64 leads on the entire ventricular surface and on the epicardial patch 5 × 5 mm. The ARI and RT dispersions were greater than AT dispersion on the entire ventricular epicardium but not in the local patch. Both ATs and ARIs were inversely related to each other in the patch but not necessarily on the entire epicardium. The RT sequence depended on the AT sequence within the local patch but not on the entire surface. The contributions of the AT sequence and ARI distribution to the RT sequence was determined by the AT/ARI dispersion ratio. Thus, the AT sequence strongly influenced the RT sequence of the local epicardial fragment but not of the entire ventricular surface. © 2011 Elsevier Inc. All rights reserved.

Aims To determine the repolarization duration gradients in different ventricular regions at atrial and ventricular pacing and to test the hypothesis that acute haemodynamic response to ventricular pacing is related to the lead position with respect to repolarization gradients. Methods and results Repolarization durations estimated as activation-recovery intervals (ARIs) were measured from unipolar electrograms recorded in the subepicardial (Epi), mid-myocardial (Mid), and subendocardial (Endo) layers of the apical and basal parts of the right ventricle (RV) and left ventricle (LV) of 15 healthy dogs under atrial and ventricular pacing. Cardiac haemodynamic variables were measured as well. At atrial pacing, ARIs were shorter in Epi than in the innermost layers (P< 0.05) in the RV apex and LV base, but not in the LV apex and RV base. Activation-recovery intervals increased from apex to base and from base to apex in RV and LV, respectively (P ≤ 0.05). At apical or basal pacing of RV and LV, repolarization gradients decayed. The dispersion of repolarization increased at LV apical pacing and preserved at RV apical pacing. The pump function of a ventricle was altered dramatically at pacing of the area with the shorter ARIs and to a lesser degree at pacing of the area with the longer ARIs (P ≤ 0.051). Conclusion The transmural and apicobasal differences in repolarization durations were heterogeneously distributed at atrial pacing. The acute haemodynamic response of the individual ventricle was better with pacing of the region with the longest repolarization suggesting a promising criterion for the lead position selection on the basis of ARIs measurements. © The Author 2011.
