The aim of the investigation was to assess the potential of cluster analysis as an additional method of data analysis for 24-hour blood pressure monitoring (BPM) in patients with both normal geometry and with various types and extents of remodeling of the left ventricle (LV).Materials and Methods. The investigation included 71 patients, ranging in age from 23 to 71. The inclusion criterion was significant arterial hypertension (AH), while exclusion criteria were symptomatic AH and severe co-morbidity. Body mass, height, waist measurement, body mass index, lipid profile, and glycemic level were determined for each subject in addition to carrying out echocardiography and conventional and cluster analysis of 24-hour BPM data of each. Results. In patient groups with different types of left ventricular hypertrophy (LVH), the conventional analysis demonstrated differences in the standard 24-hour BPM parameters. Development of concentric LVH is associated with the highest average day-time and average night-time blood pressure, pressure-induced loads and blood pressure variability. Eccentric LVH has a pathogenetic link to other factors and is formed under conditions of relatively low blood pressure. The use of cluster analysis allowed to reveal the increased occurrence of systolic-diastolic AH in concentric LVH, and isolated systolic AH and isolated diastolic AH in eccentric LVH. Conclusion. Such an integrated approach to the interpretation of 24-hour BPM results, comprising both conventional and cluster analysis, allows for objectification of the study results and reveals the significant features of AH in patients with different types of LV remodeling. © 2015, Nizhny Novgorod State Medical Academy. The aim of the investigation is to assess the efficiency of Tenoxicam epidural administration, and represent pathophysiological substantiation of new techniques of dorsalgias treatment. Materials and methods. There have been examined 75 patients with intense lumbar pain syndrome who underwent epidural pharmacotherapy of pain syndrome. The 1st group (n=50) had epidural Tenoxicam introduction, by 20 mg in 10-20 ml of saline solution, the control group (n=25) was given the combination of corticosteroids and local anesthetics. The pain syndrome dynamics in the process of treatment was assessed using visual-analog scale and qualitative and quantitative method of pain syndrome assessment. Results. The concept of degenerative cascade in dorsalgia pathogenesis has been represented. Short- and long-term treatment results of intense lumbar pain syndrome by Tenoxicam epidural introduction have shown its efficiency. Compared to the use of combination of local anesthetics and glucocorticosteroids, the present method reduces the period of treatment of patients with dorsalgias, and contributes to marked regress of pain syndrome and prolonged remissions. The aim of the investigation was to evaluate the clinical effect of URO-HYAL, a hyaluronic acid preparation, in the treatment of dysuric disorders and prevention of complications after transurethral endoscopic surgery for benign prostatic hyperplasia (BPH).Materials and Methods. A randomized study was carried out in two groups of patients operated for BPH using transurethral enucleation with bipolar and bipolar transurethral resection of the prostate. In the postoperative period, 51 patients of the main group underwent 6 intravesical instillations of URO-HYAL, 150 patients of the control group received standard antibiotic therapy. The clinical effect was assessed 1, 3, 6,12 months after the surgery on the basis of the complex urological examination results and adenoma bed bioptate histology.Results. Histological examination of the adenoma bed bioptate of patients after endoscopic surgery for BPH has shown that intravesicaladministration of URO-HYAL in the postoperative period enables almost 2.5-fold reduction of the wound surface epithelization period. One month after the surgery there have been significantly less manifestations of dysuric disorders, higher life quality and no late postoperative cicatricial complications in the main group as compared to the control.Conclusion. Treatment with URO-HYAL preparation after endoscopic surgery for BPH has been proved by our investigations to be pathogenically reasonable, clinically effective, and especially important for patients with a complicated course of BPH. © 2015, Nizhny Novgorod State Medical Academy.
