Alzheimer's disease is the most common and of the highest prevalence rate type of dementia worldwide. The amount of Alzheimer's patients in China reached 5.69 million in 2010, and gradually increased yearly. In recent decades, with the power construction even more widespread and intense worldwide, the impact of the environmental electromagnetic fields especially extremely low frequency electromagnetic fields on the risk of Alzheimer's disease has become a public health problem of governments and aroused public common concern. We summarized the epidemiology and laboratory results, outlined the effect of extremely low frequency electromagnetic fields on the risk of Alzheimer's disease and its possible mechanism. Currently, most studies focused on extremely low frequency electromagnetic fields (ELF-EMFs) increasing the risk of AD. However, because of the characteristics of ELF-EMFs and inherent defects of epidemiological investigations, the existing research results are inconsistent, and some even contradictory each other. Future research should seek to exclude experimental interference factors, establish the proper control, and reduce the influence of bias and other factors. Progressive memory impairment is the main clinical manifestations of AD, senile plaques and neurofibrillary tangles are the two major pathological features of AD, whose main component is amyloid protein(Aβ) and hyperphosphorylation of Tau protein respectively. On the one hand, calcium homeostasis and metabolism of free radicals which promote each other in the pathogenesis of AD can lead to Aβ and/or hyperphosphorylation of Tau protein overproduction and accumulation. On the other hand, ELF-EMFs can induce overproduction of the intracellular oxygen radicals or prolong its half-life, and then lead to intracellular reactive oxygen species(ROS) concentration; ELF-EMFs can interfere with calcium homeostasis and then lead to an increase in intracellular Ca2+ concentration. Hence, ELF-EMFs may induce central nervous system neuronal calcium homeostasis and metabolism of free radicals disorder, resulting in the overproduction of Aβ and/or hyperphosphorylation Tau protein and abnormal deposition in brain. In order to systematically evaluate the relationship between electromagnetic radiation exposure during pregnancy and adverse birth outcomes, by searching the databases such as PubMed, Embase, Cochrane library, CNKI and Wanfang databases, we collected case-control studies about the relationship between electromagnetic radiation exposure during pregnancy and adverse birth outcomes. The studies were screened by two independent researchers according to the inclusion and exclusion criteria. After extracting data and evaluating qualities of the included studies, RevMan 5.3 software was used to calculate odds ratio (OR), 95% confidence interval (95%CI), and probability P. The results of meta-analysis show that, during pregnancy, the usage of computers (OR is 1.57, 95%CI is 1.27 to 1.93, P<0.000 1), microwave ovens (OR is 1.69, 95%CI is 1.11 to 2.57, P=0.01), copying machines (OR is 2.09, 95%CI is 1.07 to 4.10, P=0.03), induction cookers (OR is 2.18, 95%CI is 1.33 to 3.59, P=0.002) and living or working near the electromagnetic radiation source (OR is 1.88, 95%CI is 1.28 to 2.78, P=0.001) during pregnancy are associated with increased risk of adverse birth outcomes.However, the usage of mobile phones, cordless telephones, hair dryers and electric blankets during pregnancy and working with VDT during pregnancy is not associated with the increased risk of adverse birth outcomes. These Meta-analysis results reveal that electromagnetic radiation exposure during pregnancy is probably associated with the increased risk of adverse birth outcomes. © 2016, High Voltage Engineering Editorial Department of CEPRI. In order to promote the in-depth and regular communication and cooperation between the peers in the biological electromagnetic research field and the safety evaluation research field, on the basis of the present works, epidemiological data, and laboratory results, we summarized the research progress in bio-electromagnetic technology as well as the related works of health risk evaluation. Regarding to the medical applications, μs/ps/ns pulsed electric fields have a wide application prospect in non-invasive cancer treatments, low-frequency electromagnetic fields can promote the growth of bone cells and accelerate bone reconstruction, the holmium laser lithotripsy in vivo has certain advantages for its relatively short operation time and small body damage, and the electret exposure provides a new method for clinical treatment. As for the health hazard assessment of electromagnetic exposure, there is no conclusion that whether microwave radiation poses health risks. Although there is still a large number of international controversy, in 2007 the World Health Organization announced that the evidence of associated carcinogenicity in human of power-frequency electromagnetic field inducing childhood leukemia was limited. How power-frequency electromagnetic field exposure affects animal behavior cognition is associated with the electromagnetic field's magnetic flux density and exposure duration. Under certain conditions, THz electromagnetic waves can destroy the double helix structure of DNA, and weak radiation of THz waves can interfere with the identification process of proteins. Being set in the magnetic resonance imaging field which has intensity above 4.0 T, most people will experience headaches, chest tightness, and other cardiovascular symptoms. Using the basic theories of public health, physiology, epidemiology and management of toxicology and some other disciplines, we proposed the ideas and measures for establishing the assessment system of health risks, i.e. from the four aspects including hazard identification of bio-electromagnetic technology, hazard characterization, exposure assessment, and risk characterization, system evaluation will be proceeded on the basic principles of bio-electromagnetic applications and related health risks of electromagnetic exposure, for the reasonable applications of bio-electromagnetic technologies and preparing preventive measures to avoid possible health risks. The first step should be establishing a scientific, standardized, multi-disciplinary integrated health risk assessment system to electromagnetic bio-effects. ©, 2015, Science Press.
