This study is to investigate the influencing factors of benign nurse-patient relationship cognitive differences between nurses and patients, and provide the basis for building benign nurse-patient relationship. The self-designed questionnaire and a random sample was used in this study. Then the influencing factors of benign nurse-patient relationship cognitive differences between nurses and patients were analyzed. Qualifications and seniority affect the nurses' cognitive, while payment methods and education affect the patient's cognitive. According to the demand for nurses and patients, some measures should be taken to improve benign nurse-patient relationship, such as, training at different levels, optimizing service processes and content, and strengthening the nurse-patient communication.

Objective: To investigate CT and digital subtraction angiography (DSA) features of liver involvement in hereditary hemorrhagic telangiectasia (HHT). Methods: Six patients of HTT with hepatic involvement, who were encountered at the hospital during the period from Sep. 2005 to Oct. 2012, were enrolled in this study. The clinical data as well as CT and DSA findings were retrospectively analyzed. Results: Enhanced CT scanning showed that on arterial phase tortuous and dilated hepatic artery and its branches, as well as early visualization of thickened hepatic and portal veins could be seen. Multiple focal and small patchy enhancement of different sizes were revealed in liver. On portal phase some lesions showed continuous enhancement, meanwhile the liver parenchyma became enhanced and hepatic arterial density decreased and the portal as well as the hepatic veins stood out more clearly. Computed tomography angiography (CTA) could demonstrate the anatomical variations of hepatic artery. On DSA the lesions were manifested as thickened and tortuous hepatic arteries with diffuse massive staining in liver parenchyma and early filling of the hepatic veins. Besides, thickened portal stem and its intrahepatic branches, abnormal hepatic artery as well as superior mesenteric artery steal syndrome could also be found. Conclusion: On enhanced CT scanning and DSA, liver involvement in hereditary hemorrhagic telangiectasia has characteristic features, which are very helpful in diagnosing this disease.

To study the expression of p38 Mitogen Activated Protein Kinase (MAPK) and Intercellular Adhesion Molecule-1 (ICAM-1) in LPS-induced Acute Lung Injury (ALI) and the intervention effect of resveratrol. Thirty two male Sprague-Dawley rats were randomly assigned to four groups (n = 8): saline group, resveratrol alone, Lipopolysaccharide (LPS) alone and resveratrol pretreatment (LPS + resveratrol). The rats were treated with resveratrol (5 mg kg -1, intraperitoneal injection (i.p) or saline, 30 min prior to LPS administration (6 mg kg -1, intravenous injection). About 6 h after LPS injection, samples of pulmonary tissue were collected. Blood gas analysis was carried out Wet/Dry ratios (W/D), Myeloperoxidase (MPO) activity, albumin in BALF and Tumor Necrosis Factor-alpha (TNF-α) and Interleukin-6 (IL-6) levels in serum were measured. Optical microscopy was performed to examine pathological changes in lungs and lung injury score was assessed. The pulmonary expression of p38 MAPK was analyzed by western blot. RT-PCR analysis was performed in lung tissue to assess gene expression of ICAM-1. PaO 2 in the lipopolysaccharide group significantly decreased compared with that in the saline group (p<0.01). The W/D ratio increased significantly in the LPS group (5.60±0.21) as compared with that in the saline group (4.21±0.12) (p<0.01) which was significantly reduced in the LPS + resveratrol group (4.58±0.39) (p<0.01). MPO levels increased significantly in the lipopolysaccharide group compared with that in the saline group which was reduced in the resveratrol + LPS group. The TNF-α and IL-6 level of pulmonary tissue increased significantly in the LPS group compared with that in the saline group protein (p<0.01). However, the increase of TNF-α level of serum was significantly reduced in the resveratrol + lipopolysaccharide group protein (p<0.01). The lung tissues from the lipopolysaccharide group were significantly damaged which were less pronounced in the resveratrol + lipopolysaccharide group. The expression of p38 MAPK, phospho-p38 and ICAM-1 was up-regulated significantly in the lung after rats were injected with LPS (p<0.01). Compared with LPS group resveratrol significantly inhibited the expression of p38 MAPK and ICAM-1 in the resveratrol + lipopolysaccharide group. The expression of p38 MAPK, phospho-p38 and ICAM-1 is up-regulated in rat lung with LPS-induced ALI. Resveratrol injection has a protective effect on ALI and its therapeutic mechanism may be related to inhibition of the phosphorylation of p38 MAPK and ICAM-1. © Medwell Journals, 2012.

The lack of information concerning individual variation in drug-metabolizing enzymes is one of the most important obstacles for designing personalized medicine approaches for hepatocellular carcinoma (HCC) patients. To assess cytochrome P450 (CYP) in the metabolism of endogenous and exogenous molecules in an HCC setting, the activity changes of 10 major CYPs in microsomes from 105 normal and 102 HCC liver tissue samples were investigated. We found that CYP activity values expressed as intrinsic clearance (CLint) differed between HCC patients and control subjects. HCC patient samples showed increased CLint for CYP2C9, CYP2D6, and CYP2E1 compared to controls. Meanwhile, CYP1A2, CYP2C8, and CYP2C19 CLint values decreased and CYP2A6, CYP2B6, and CYP3A4/5 activity was unchanged relative to controls. For patients with HCC accompanied by fibrosis or cirrhosis, the same activity changes were seen for the CYP isoforms, except for CYP2D6 which had higher values in HCC patients with cirrhosis. Moreover, CYP2D6*10 (100C>T), CYP2C9*3 (42614 A>C), and CYP3A5*3 (6986A>G) polymorphisms had definite effects on enzyme activities. In the HCC group, the CLint of CYP2D6*10 mutant homozygote was decreased by 95% compared to wild-type samples, and the frequency of this homozygote was 2.8-fold lower than the controls. In conclusion, the activities of CYP isoforms were differentially affected in HCC patients. Genetic polymorphisms of some CYP enzymes, especially CYP2D6*10, could affect enzyme activity. CYP2D6*10 allelic frequency was significantly different between HCC patients and control subjects. These findings may be useful for personalizing the clinical treatment of HCC patients as well as predicting the risk of hepatocarcinogenesis.

OBJECTIVE: To investigate the effects of adefovir dipivoxil (ADV) on blood phosphorus metabolism in patients with chronic hepatitis B (CHB).METHODS: Patients with hepatitis B surface antigen (HBsAg)-positive CHB were treated with ADV alone, ADV combined with interferon (IFN), or ADV combined with lamivudine (LAM). Changes in levels of calcium, phosphate, urea, and creatinine were assessed at treatment weeks 4, 12, 24, 48, 72 and 96. Statistical analysis was carried out with SPSS 16 software; influential factors were analyzed by ANOVA and non-conditional logistic regression analysis.RESULTS: During the course of treatments, 32 (42.6%) of the patients presented with low phosphorus. The highest incidence of low phosphorus was found to have occurred at treatment week 24 (25.0%, 27.5% and 36.4% respectively, with no statistical difference between three groups, x2=0.225, P>0.225). Patients with hypophosphatemia did not show a significant difference in serum phosphorus levels from the other patients (F=1.853, P=0.169). Logistic regression showed a correlation between low phosphorus and sex (x2=7.876, P<0.05), age (t=2.479, P<0.05), and serum creatinine (t =-2.256, P<0.05), but not with blood urea nitrogen or blood calcium (P>0.05).CONCLUSION: ADV antiviral treatment can decrease the blood phosphorous levels of CHB patients, particularly over extended time of treatment, and the occurrence of low phosphorus is more common than of mild phosphorus decrease.Male and elderly patients may be at greater risk of this complication. The incidence and severity of low phosphorus is not significantly different for the different ADV-based treatment regimens.

Objects: The study is aim to understand the cognitive differences of nurse-patient relationship between nurses and patients, and provide the basis for the implementation of targeted continuing nursing education. Methods: The self-designed questionnaire was used, which included the current characteristics, development trends and future expectations of nurse-patient relationship, and the responsible subject and reasons of discordant nurse-patient relationship. Results: There were significant cognitive differences between nurses and patients about the current characteristics, development trends and future expectations of the nurse-patient relationship. Revelation: It's necessary to set up the continuing education content to meet the needs of clinical nurses and patients, to increase nurses' skills training about the knowledge of Psychology and etiquette, to strengthen nurse-patient communication, to eliminate both asymmetric information, so as to establish harmonious nurse-patient relationship-oriented nursing continuing education system.
