Generation of thrombin is pivotal step of hemostasis, according to in vitro experiments, thrombin generation occurs in 2 phases. Initially small amount of thrombin is produced after activation of factor X by complex TF / FVIIa. Thrombin multiplies coagulation by activation of platelets, F V a F VIII. Complex of TF / FVIIa activates also F VIII a, F IX, F VIII and FIXa and its binding to surface of platelets, which activates F X and products quantity of thrombin and leads formation of fibrin and finaly to triggering clotting.

Extramedullary (EM) manifestation of acute myeloid leukemia (AML) is relatively rare, it occurs frequently in 2 forms: myeloid sarcoma (MS) or leukemia cutis (LC). The molecular mechanism of EM involvement has not been clearly explained yet, but it is obvious that homing of leukemic cells to peripheral tissues is controlled by chemokine receptors, adhesion molecules and their interactions. The isolated EM relapse (IEMR) after allogeneic haematopoietic stem cell transplantation (ASCT) is an uncommon clinical manifestation with an unfavourable prognosis, especially in case of systemic relapse. It is probably the result of GvL imbalance in the bone marrow and peripheral tissues. The treatment of IEMR is not clearly defined. We present a case report of the rapid EM involvement in both breasts in patient with acute leukemia (mixed phenotype) (MPAL) after ASCT. Verification biopsy under ultrasound-guidance confirmed the presence of leukemic infiltration. The aim of this work is to show the less common case of IEMR with emphasis on comprehensive diagnosis and a very good therapeutic response after discontinuation of immunosuppression.
