The aim of the present study was to develop sustained release (SR) matrix tablet of ketorolac tromethamine using natural polymer xanthan gum with cellulose derivative (sodiumcarboxy methyl cellulose) and Polyvinylpyrrolidone K30 for effective management of pain and inflammation in spinal injuries. The formulated tablets were evaluated for uniformity of weight, drug content, friability, hardness, thickness, swelling index, in vitro drug release and in vivo analgesic and antiinflammatory activity. The physicochemical properties of tablets were found within the limits. The SR matrix tablets of ketorolac showed good swelling (increased with xanthan gum’s concentration), drug content and the dissolution profile. The drug released increased with decrease in swelling properties. The formulation with 20% w/v of xanthan showed the lowest % drug release (49.6%) at the end of 8 hrs. It was concluded that increasing the concentration of gum from 5% w/v to 20% w/v in the formulation decreased the amount of drug release from the tablet. The formulation with 10% xanthan gum found to be the best in % pain reduction (54.6%) and % inhibition of paw edema (51.58%). The xanthan gum based SR matrix tablets of ketorolac tromethamine can be explored for potential therapeutic uses in spinal injuries. © 2015 American Scientific Publishers. All rights reserved. To isolate exosomes from rabbit aqueous humor and to investigate their immunosuppression function. Aqueous humor was collected from 40 New Zealand rabbits and exosomes were isolated by fractional separation and ultracentrifugation methods; the morphology was studied with electron microscopy. The immunosuppressive-related proteins of exosomes were detected with Western blotting; their inhibitory effect on ConA-induced proliferation of T lymphocyte was estimation with CCK-8 cells proliferation assay. Eight milliliters of aqueous humor were collected from 40 New Zealand rabbits and 200 μg exosomes was yielded. Under electron microscope, the exosomes had typical structure of lipid bi-layer with a diameter of 50-100 nm. The results of Western blotting showed that these exosomes expressed Hsp70, CD9 and Alix but not Grp94, presenting a typical exosomes protein profile. Moreover, exosomes expressed high level of TGF-β and significantly inhibited the proliferation of T lymphocytes. Immunosuppressive exosomes can be isolated from rabbit aqueous humor, which may be involved in immunotolerance of the eye. Source

Background: To improve the estimation of tumor status and facilitate the stage-dependent treatment planning, we developed a reliable and easy-to-use risk score for prediction of tumor-node-metastasis stages in gastric cancer. Methods: Clinicopathological data were collected prospectively from 108 curatively resected patients with gastric cancer. The risk score was established on the basis of independent predictive factors for tumor stages, and its performance was evaluated by receiver operating characteristic (ROC) analysis. Results: The following 4 independent factors were included in our score: serum albumin levels, tumor size, T and N categories determined by helical computed tomography. Using ROC analysis, we chose a score at 7 as the optimal cut point for differentiating the more advanced disease (stage III/IV) from the less advanced one (stage I/II). With the deflned cut point, our score allowed predicting stage III/IV with sensitivity of 79.6%, speciflcity of 85.2%, and accuracy of 82.4%. The discriminative ability of this score was good (the area under the ROC curve, 0.861-0.965). Conclusions: The risk score may be helpful to preoperative gastric cancer staging. It probably assists surgeons in deciding the extent of surgery and in choosing the appropriate perioperative adjuvant therapies. Copyright © 2011 by Lippincott Williams & Wilkins. Source

To retrospectively analyze the clinical characteristics of the benign paroxysmal positional vertigo (BPPV) secondary to the sudden deafness (SD) and to explore pathogenetic mechanism. One hundred and seventy-eight cases of the SD in our department were retrospectively analyzed. They were all treated under the guidance of clinical guidelines. (1) In all these patient's with SD, there were 31 cases with BPPV secondary to the SD. There were 26 cases of BPPV of posterior semicircular canal and 5 cases of BPPV of lateral BPPV semicircular canal. All patients with BPPV were diagnosed as the same ears as the SD, including 16 cases on left sides and 15 on right sides. (2) The interval between the onset of SD and BPPV was less than one week in 27 cases, between one week and one month in 3 cases, and between one and three months in 1 case. (3) All patients with BPPV secondary to the SD were cured with Epley maneuver or Barbecue roll maneuver. The occurrence of BPPV may follow SD, and the major of BPPV secondary to the SD occurs in the posterior semicircular canal. The canalith repositioning is an effective therapy to the secondary BPPV. Objective: To explore the clinicopathologic features, diagnosis and prognosis of breast apocrine carcinoma. Methods: Breast apocrine carcinoma was diagnosed by morphology and detection of ER, AR, GCDFP-15 with immunohistochemistry, and the literatures were reviewed. Results: Breast apocrine carcinoma had special pathological changes. The tumor was almost completely composed of apocrine cells, there were more high negative rates of ER and PR and positive AR in immunohistochemistry. Conclusions: Breast apocrine carcinoma is a rare kind of breast tumors. It is characterized by pathologic changes and better prognosis, but there is no specific clinic feather. Its diagnosis mainly relies to morphology. Source
