A general practitioner builds an informal network of medical care composed of specialists correspondents, hospitalization structures, and medical!social auxiliaries. The key determining factors of all these correspondents are reputation, geographical and timely accessibility, quality of the communication and information feedback, absence of competition and decisional collaboration. Patient's desires and quality of the relationship they build with these correspondents are also very key factors. This informal network, in addition to other institutionalized structures, makes the quality and originality of each physician's practice.

The enhanced platelet aggregation and thrombosis occurring in TTP is probably due to an unbalance between agents insulting endothelial integrity and natural antithrombotic factors, such as NO. Using a sensitive and specific HPLC assay, we tested the hypothesis of NO involvement in TTP, comparing NO production, as the stable end-products nitrites and nitrates, in the plasma of 29 TTP patients and of 29 healthy subjects matched for sex and age. Average nitrate titer was 25.868 M/L in the TTP group vs 24.234 M/L in the control group (p = n.s.), while nitrite were undetectable in both groups. Moreover, nitrate titers did not correlate with hemoglobin value, platelet count, LDH values, or with Rose and Eldors severity score. In conclusion, even though the enhanced platelet aggregation observed in TTP could be due to reduced natural antithrombotic substances, NO involvement in the pathogenesis of TTP appears unlikely.

The first meta-analysis of randomozed control trials (RCTs) in patients "at high-risk of cardiovascular disease", reporting cardiovascular outcomes either as efficacy or safety events, states that influenza vaccine was associated with a lower risk of major adverse cardiovascular events. On the contrary, the published data show that the vaccine was associated with a lower risk of cardiovascular events only among patients with more active coronary artery disease (CAD). In patients with stable CAD the vaccine did not show a significant reduction in risk of composite cardiac events. In people without CAD the vaccine showed a tendency for increased risk of adverse cardiovascular events, cardiovascular mortality and all-cause mortality. The limitations of the published data do not allow definite conclusions, but these alarming results should now require a large, well designed randomized controlled trial before relaunching indiscriminate vaccination campaigns.

The aim of this study was to compare the results obtained using an electrothermal bipolar vessel sealing system (Ligasure Precise), a harmonic curved shears (Harmonic Focus) and traditional technique in total thyroidectomy. We have enrolled 93 patients and assigned randomly to three groups of 31 pt: groups L (Ligasure Precise), F (Harmonic Focus) and C (traditional thecnique). Recorded data were demographics, preoperative serum calcium levels, operation time, length of hospital stay, weight of exported gland and pathology, postoperative calcemia at one and two days and recurrent laryngeal nerve paralysis. The three groups did not present statistically significant differences in term of age, gender and pathology classification. No postoperative haemorrhages were observed. The overall incidence of hypocalcemia was 38.9% (36 pt) and the mean days of hospitalization were 2.3 days without statistically significant differences between the three groups. Only one patient (group F) presented temporary recurrent laryngeal nerve paralysis. Mean operation time (minutes) was significantly reduced by approximately 15% in group F (62.7+/-14.1) compared with group C (72.7+/-13.6; Kruskal-Wallis test: p<0.05). Both devices resulted safe and efficient. The only advantage observed was a significant reduction operation time when using Harmonic Foscus curved shears compared to the other techniques.

With a length of 35 km and more than 100,000 people living along its route, the Paris ring road (known in French as "boulevard périphé rique") is a recurring source of problems, especially as regards noise pollution. A victim of its own success, congested daily at certain times, the popular "périph" caters for a quarter of Parisian journeys and provides a major link between Paris and the neighbouring communities. The ring road has daily traffic of some 300,000 vehicles. In order to complete the strategic noise maps produced by the Paris City Council and the 21 adjoining communities belonging to the departments of Hauts-de-Seine, Seine-Saint-Denis and Val-de-Marne, in spring 2009 the company Bruitparif conducted a major series of measurements. The core of the survey deployment was eight fixed stations taking 24h readings, second by second, of the noise generated by traffic at the roadside and in nearby dwellings on either side of the ring road. Finally a laboratory vehicle, fitted with innovative technologies and autonomous in energy terms thanks to built-in solar panels, took some fifty one-hour measurements, i.e. at one point in each 700m stretch.
