Objective: To explore the role of clinical thinking in pharmaceutical practice hy clinical pharmacists. Methods: By using clinical thinking, clinical pharmacists analyzed and identified several typical eases and carried out a series of pharma ceutical practice. Results: The implementation of pharmaceutical practice has achieved the following positive results; more indi vidualized clinical treatment, safer medication, more rational and economical application of drugs. Conclusion; Clinical thinking could play an important role in the implementation of pharmaceutical practice by clinical pharmacists.

Skewed X chromosome inactivation (XCI) is a rare reason for hemophilia B in females. It is indefinite whether X chromosome reactivation (XCR) would occur when cells of hemophilia B patients with skewed XCI were reprogrammed into induced pluripotent stem cells (iPSCs). In this study, we investigated a female hemophilia B patient with a known F9 gene mutation: c.676C>T, p.Arg226Trp. We demonstrated that skewed XCI was the pathogenesis of the patient, and we successfully generated numerous iPSC colonies of the patient from peripheral blood mononuclear cells (PBMNCs), which was the first time for generating hemophilia-specific iPSCs from PBMNCs. Then we detected the XCI state of these iPSCs. Ninety-two iPSC lines were picked for XCI analysis. All of them retained an inactive X chromosome, which could be proved by amplification of the androgen receptor gene and XIST (X inactivation-specific transcript), expression of H3K27me3, and existence of XIST clouds in XIST RNA fluorescence in situ hybridization (FISH) analysis. We attempted to obtain iPSC lines with the wild-type F9 gene on the active X chromosome for further disease treatment. But it turned out that the patient's iPSCs were still skewed such as the somatic cells with 92 iPSC lines having mutant F9 on the active X chromosome. In conclusion, skewed XCI is one reason for hemophilia in females. PBMNCs are excellent somatic cell resources for hemophilia patients to do reprogramming. More attentions should be paid to generate naive iPSCs with two active X chromosomes for further clinical disease treatment. Background: We performed a meta-analysis to compare the diagnostic performance of integrated 18FDG PET-CT with that of 18FDG PET alone and CT alone for the overall assessment of tumor distant metastases. Methods: We performed a meta-analysis of 11 available articles (1107 patients). We calculated sensitivities, specificities, positive likelihood ratios, and negative likelihood ratios for integrated PET-CT, PET alone, and CT alone, respectively. We also constructed summary receiver operating characteristic curves for integrated PET-CT, PET alone, and CT alone, respectively. Results: Across 10 comparative studies (1058 patients), integrated PET-CT has higher sensitivity (0.95 vs 0.85) and similar specificity (0.96 vs 0.95) with PET alone. Across 7 comparative studies (745 patients), integrated PET-CT has higher sensitivity (0.97 vs 0.80) and similar specificity (0.97 vs 0.94) with CT alone. Conclusion: Integrated PET-CT is the most sensitive and accurate modality for tumor distant metastasis staging. Integrating PET with CT may tend to improve the sensitivity than PET alone and CT alone. © 2013 Elsevier Ltd. All rights reserved.

The unchecked overproduction of reactive oxygen and nitrogen species by inflammatory cells can cause tissue damage, intensify inflammation, promote apoptosis, and accelerate the progression of immune-mediated glomerulonephritis (GN). Here we tested whether the anti-inflammatory and antioxidant properties of the green tea polyphenol ()-epigallocatechin-3-gallate (EGCG) favorably affect the development of immune-mediated GN. Pretreatment of 129/svJ mice with EGCG from 2 days before to 2 weeks after the induction of GN led to reduced proteinuria and serum creatinine, and marked improvement in renal histology when compared with vehicle-pretreated diseased mice. This pretreatment reduced oxidative stress, and normalized osteopontin, p65/nuclear factor-B, inducible nitric oxide synthase, nitric oxide metabolites, p-Akt, phosphorylated extracellular signal-regulated kinases 1 and 2, p47phox, and myeloperoxidase, all of which were elevated in vehicle-pretreated diseased mice. Levels of glutathione peroxidase and peroxisome proliferator-activated receptor-γ (PPARγ), both reduced in the vehicle-pretreated diseased mice, were normalized. This renoprotective effect was reversed by concomitant administration of the PPARγ antagonist GW9662 throughout the EGCG pretreatment period. Importantly, mortality and renal dysfunction were significantly attenuated even when the polyphenol treatment was initiated 1 week after the onset of GN. Thus, EGCG reversed the progression of immune-mediated GN in mice by targeting redox and inflammatory pathways. © 2011 International Society of Nephrology.

S1P (sphingosine 1-phosphate) is one of the metabolite products of membrane sphingomyelin, and it plays an important role in formation, transformation and progression of many malignant tumors as well as the regulation of cell proliferation, apoptosis and angiogenesis. Furthermore, S1P plays different roles in regulation of biological behaviors of malignant tumors depending on the types of the cells and S1PR (sphingosine 1-phosphate receptor). For ovarian cancer and colon cancer, S1P improves the growth of tumors cells by S1PR1/S1PR3, but for malignant melanoma, it inhibits the cell growth by S1PR2. There is a difference in the downstream pathways mediated by difficult receptors. This review summarizes the advances in research on the roles of S1P in regulating biological behaviors of tumor cells and different signal pathways of S1P mediated by various receptors. To assess the characteristics of the dysfunction of semicircular canal in benign paroxysmal positional vertigo and the relationship with the ectopic otoconia. There were 214 patients with benign paroxysmal positional vertigo (BPPV), including 107 cases of posterior semicircular canal canalithiasis (PSC-Can) 80 cases of horizontal semicircular canal canalithiasis (HSC-Can), 27 cases of horizontal semicircular canal cupulolithiasis (HSC-Cup). One hundred and ninety (88.8%) patients were accompany with relevant diseases while 24 (11.2%) cases were not. They accepted low, middle and high frequency vestibular function tests, including caloric test (CT), head shaking test (HST) and video head impulse test (vHIT) respectively. The parameters of the unilateral weakness (UW), head shaking nystagmus (HSN) and video head impulse test gain (vHIT-G) were observed. Patients classified into three groups (PSC-Can, HSC-Can, HSC-Cup) according to the involvement semicircular canal. The results of the three tests were analyzed with SPSS16.0 software. The positive cases of the three tests were vHIT: 15 (7.0%), HST: 52 (24.3%), CT: 152 (71.0%), a statistically significant difference (P < 0.05) was found between the three tests. When compared the Caloric Test, HST and vHIT between the BPPV patients with and without relevant diseases, there were no significant differences (P > 0.05). The variance without statistical significance (P > 0.05) was showed between three tests' results in each groups, it was also showed that the variance between the three groups in each tests reached no statistical significance (P > 0.05). The test of affected side UW between PSC-Can, HSC-Can and HSC-Cup showed the variance without statistical significance (F = 0.970, P = 0.383). The lesion of semicircular canals has the same etiological factors with the utricle pathological change in benign paroxysmal positional vertigo, and the dysfunction mostly happens in low frequency range of semicircular canal frequency band. The ectopic otoconia is not the main etiological factors for that. HST and vHIT of middle and high frequency can not be ultimately used for the screening test evaluating due to the semicircular canal function in BPPV.

Serologic characters of Bx subtype were analyzed with standard methods, PCR-SSP method was used for its genotype, and DNA sequences of 6 and 7 exons of ABO genes were analyzed by DNA sequencing. Serologically, the anti-B test was no or 1 + , with anti-AB ( 1 + or 2 + ). The patient's auto antibodies were negative. PCR-SSP showed the patient was Bx02/02. ABO genetic locus sequencing was 261delG in exon 6 and 297 was GG homozygous. The sample was a combination of blood group B and O2 because 297A > G, 526C > G, 657C > T, 703G > A, 796C > A, 803 G > C, 905 A > G, 930A > G shown by direct DNA sequencing and 803 G > C, 905 A > G shown by cloning DNA sequencing. The serological phenotype of the specimen was B and the genetic sequence was a combination of ABO*Bx. 02. 1. 1 and ABO*021. 01. 1. 1.

BACKGROUND: Basic fibroblast growth factor is a pluripotent cytokine that can promote the proliferation of mesodermal and neuroectodermal cells. OBJECTIVE: To observe the effect of basic fibroblast growth factor in human periodontal ligament cells cultured in vitro. METHODS: Human periodontal ligament cells at passage 5 were inoculated into the 96-well plates at the density of 1×108/L, and were randomly divided into four groups. The cells were cultured in α-MEM containing 15% fetal bovine serum and 0, 1, 10, 100 μg/L basic fibroblast growth factor, respectively. At 1, 3, 5, 7 days of the culture, the cell proliferation was determined, and the activity of alkaline phosphates was detected at 1 and 7 days. RESULTS AND CONCLUSION: There were significant differences in the proliferation of human periodontal ligament cells among the four groups (F=6.586, P=0.024). As the increase of the basic fibroblast growth factor concentrations, the absorbance value was gradually increased and reached the peak in 100 μg/L basic fibroblast growth factor group (P < 0.05). The alkaline phosphatase activity in basic fibroblast growth factor groups was lower than that of the control group (P=0.000), the higher the concentration was, the lower activity was (P < 0.05). Results show that basic fibroblast growth factor can promote the proliferation of human periodontal ligament cells and inhibit the activity of alkaline phosphatase, and the effect is concentration-dependent.

To explore the epidemiological profiles of invasive fungal disease (IFD) in hospitalized patients with hematological diseases during 2007-2012.A total of 419 IFD patients with hematological diseases from January 2007 to December 2012 were reviewed. All of them were analyzed with regards to diagnostic levels, infection sites and various related factors.(1) A total of 233 cases (55.61%) were preliminarily identified as IFD, 140 cases (33.41%) had a clinical diagnosis and 46 cases (10.98%) were confirmed cases of IFD. (2) Among 46 confirmed cases of IFD, there were agranulocytosis (n = 43) and aspergillosis infection (n = 36). (3) Respiratory tract was the most frequent infection site in all IFD patients (85.20%) . (4) And chemotherapy-induced agranulocytosis was a major reason for IFD patients with hematological diseases. The number of IFD patients without chemotherapy had a rising trend. (5) The age group of IFD was during 41-60 years old. (6) All of them stayed on antibiotic therapy at the diagnosis of IFD. The numbers of antibiotics were two(205 cases, 48.93%) and three(179 cases, 42.72%). (7) The peak incidence of IFD was recorded in January, July and December. And June was another lower peak.Agranulocytosis is the main reason for IFD patients with hematological disease. The data is important and valuable for the early diagnosis and therapy of IFD patients with hematological disease.
