Anemia is the most common pathology encountered in hematology. Etiologies are numerous, so it is important to adopt a rigorous approach. Complementary examinations must be specific to the clinical situation in order to determine the mechanisms on the one hand and decide the therapeutic management on the other. We report the observation of a case of sudden onset of profound pancytopenia. Investigation led to the diagnosis of major folic acid deficiency with favorable evolution. Through this case, we describe the diagnostic approach towards anemia and the mechanisms involved in the formation of folate deficiency. Summary Diagnosis of lower respiratory tract infections may be difficult, that's why physicians tend to increase diagnostic tests and to quickly introduce antibiotics. Recommended only in severe pneumopathy, Legionella and Streptococcus pneumoniae rapid diagnostic tests lead to improve performance of the etiological diagnosis of pneumopathies and to promote timely adequate management. Although this tests provide major advance in the management of pneumopathies, it is important to keep in mind their limits, their place in the diagnostic strategy, and to monitor prescribing practices in order to keep a better cost efficiency. The accreditation process, according to NF EN ISO 15189, implies a prior evaluation of the new reagent on-site for the implementation of each new assay technique. Thus, our new standardized method for determination of creatinine (non compensated method) in plasma or serum on UniCel® DxC 600 (Beckman Coulter) has been tested according to LAB GTA 04 protocol. The reagent meets the quality criteria recommended by Valtec protocol, except fidelity with the low concentration standard (50 μmol/L). Besides there is no problem of results transferability with the two other techniques used in the laboratory (Jaffe compensated and enzymatic methods performed on Cobas Integra® 800). Source
