The aim of the investigation is to study the efficiency of pericardioscopy in the diagnosis and treatment of various forms of pericardial effusion.Materials and Metgods. There have been performed 38 pericardioscopies in 37 patients. The procedure has been carried out using a rigid thoracoscope, 10 mm in diameter, with 30°-optics. The quality of images of various pericardial areas has been studied in pranspleural and extrapleural pericardioscopy. And there has been assessed clinical efficiency of pericardioscopy. Results. Pericardioscopy allowed to perform direct imaging of pericardium and target biopsy of changed tissue due to which the diagnostic value of surgery increased from 48.6 to 67.6% (p=0.042). Pericardioscopy efficiency for video-assisted sanation of the pericardium in suppurative pericardites was stated. In the process of the study there were found out the advantages and disadvantages of prans- and extrapleural videopericardioscopy. There were no fatal cases. In one case the surgery was complicated by hemothorax. Conclusion. High diagnostic value, low percentage, low complication rate and the absence of lethality when using pericardioscopy allows to recommend the technique for wide use in clinical practice.

The aim of the investigation is to study the frequency indexes of heart rate variability (HRV) in three stages of stress test with physical exercise in patients with stable angina and to determine the peculiarities of their vegetative responsiveness according to cluster analysis findings. Materials and methods. Stress test with physical exercise showed the dynamics of HRV normalized spectral indexes and the peculiarities of autonomic nervous system (ANS) responsiveness during the stress test with physical exercise in 121 patients with congestive heart failure (chf) with various degrees of coronary involvement and 14 conditionally healthy people. All the patients had positive results of stress test with physical exercise using bicycle ergometer and coronary angiography findings. Results. The capacity of HRV low- and high-frequency indexes depending on coronary atherosclerosis intensity using cluster analysis was assessed. The relationship between HRV normalized spectral indexes and the severity of coronary involvement was revealed. The functional dynamic principle of vegetative regulation examination during the stress test with physical exercise is prospective to comprehend the interacting processes of sympathetic and parasympathetic parts of ANS and can be used for prognosis of coronary atherosclerosis severity in CHF patients.

The aim of the investigation is to evaluate the efficacy of the novel technique of endovascular retrograde circulatory support of the cardiac muscle according to the data obtained by studying the main functional parameters of the left ventricular myocardium in the medium terms after complete high-risk radioendovascular revascularization carried out in patients with acute coronary syndrome (ACS) with ST-elevation and non-ST-elevation and multivessel lesion of the coronary arterial bloodstream. Materials and Methods. The results of treating 24 patients with ACS and initial myocardial dysfunction have been analyzed. Group 1 (n=12) included ACS patients with ST-elevation, patients of group 2 (n=12) were with non-ST-elevation. Both groups were divided into equal subgroups: subgroup A comprised patients with retrograde perfusion circulatory support; whereas subgroup B included patients without retrograde perfusion support. Results. In group 1 complete myocardium function was observed to normalize in 7 individuals (in 6 and 1 patients in subgroups A and B, respectively) 12 months after the operation (medium term). In group 2 complete normalization of the myocardium function occurred in 6 individuals (in 4 and 2 patients in subgroups A and B, respectively) in the same period. Patients in subgroups A had significantly better parameters of local contractility and diastolic function 12 months after the operation compared to the patients from subgroups B. Conclusion. Application of the novel technique of retrograde perfusion of the left ventricular anterior wall during complete endovascular revascularization in high-risk patients with ACS results in essential improvement of the results. © 2015, Nizhny Novgorod State Medical Academy. The aim of the investigation is to study the characteristics and prognostic significance of β-adrenoreception of erythrocytes cell membranes (β-ARM) in patients with myocardial infarction (MI) with and without diabetes mellitus type 2 (DM2). Materials and Methods. There was carried out complex clinicolaboratory examination of 27 patients with MI and DM2 and 21 patients with MI and without DM2 with β-ARM quantitative assessment. Results. β-ARM index in patients with MI and DM2 was stated to be lower. It gave evidence of an increased receptor sensibility to endogenic catecholamines. β-ARM index decrease in DM2 was associated with a longer diabetic anamnesis, diabetic nephropathy, as well as the more severe MI in the form of the more frequent arrhythmias and the worst recovery potential of systolic function of the left ventricular myocardium. Decreased β-ARM indexes in patients without DM2 was also associated with the more severe MI as IV degree of MI, recurrences of early postinfarction angina pectoris and marked myocardial dysfunction.

The aim of the investigation is to determine possible predictors of myocardial infarction (MI) in patients with chronic obstructive pulmonary disease (COPD) and assess the effect of bronchopulmonary pathology on the processes of postinfarction cardial remodelling. Materials and Methods. 235 patients were examined and divided into three groups. The 1st group (n=70) consisted of patients with MI combined with COPD, the 2nd group (n=39)- MI patients, and the 3rd one (n=126)- COPD patients. To assess the effect of COPD exacerbation on cardial remodelling, the first group was subdivided into 2 subgroups: the patients without COPD exacerbation (n=35) and the patients with COPD signs (n=35). Conclusion. The presence of COPD has a negative effect on the processes of postinfarction remodelling due to the aggravation of systolic dysfunction of the left ventricle and dilatation of the heart. The exacerbation of bronchopulmonary pathology is the factor that triggers MI, as well as it can be the trigger of acute coronary events resulting from the activation of systemic inflammation.

The aim of the investigation is to assess the efficacy of various detoxication techniques in general peritonitis complicated by multiple organ failure and develop a new way of using plasmapheresis. Materials and Methods. There were studied 111 patients with general peritonitis complicated by multiple organ failure. According to the detoxication techniques used, the patients were divided into 4 groups: in the 1st group (n=26) in complex intensive care there were used indirect electrochemical oxidation of blood in the form of intravenous infusion of sodium hypochlorite; in the 2nd group (n=29) standard plasmapheresis was used; in the 3rd group (n=31) there was used combination of membrane plasmapheresis and sodium hypochlorite according to an original technique; and in the 4th group (n=25) no detoxication was used. The efficacy of detoxication was assessed by general clinical and biochemical blood values, the content of substances of low and medium molecular mass in internal environment. Results. The combination of plasmapheresis and sodium hypochlorite infused before plasmafilter leads to detoxication effect enhancement of standard plasmapheresis procedure, as well as contributes to the recovery of renal detoxicating function; and it is proved by the dynamics of endogenous intoxication markers in the compared groups.

The aim of the investigation is to determine an optimal method of glycemic control in patients with cerebrovascular accident and type 2 diabetes mellitus. Materials and methods. 43 patients with insulin and type 2 diabetes mellitus were included into the study. The patients were divided into two groups: the patients of the first group (n=23) had continuous insulin infusion within not less than 24 h, the patients of the second one (n=20) - subcutaneous insulin injections. Results. The patients of both groups had statistically significant difference of glycemic indexes in 24 h, and furthermore, the control group had high variability of glycemic indexes. Continuous insulin infusion compared to discrete subcutaneous injections provides gradual glycemia decline up to target values and more stable retention of glycemia within the given range that increases intensive insulin therapy safety. It allows to consider the method of intravenous insulin administration to be optimal for glycemic control in patients with type 2 diabetes mellitus in peracute stroke.

The aim of the investigation is to study the correlation of functional values of left ventricular myocardium and the data of subjective state of patients with chronic coronary occlusion according to echocardiography findings and a new technique of subjective state assessment of patients with incomplete myocardial revascularization. Materials and methods. The treatment results of 74 patients with coronary occlusion were analyzed. The 1st group consisted of 35 (47%) patients with chronic occlusion localized in the right coronary artery, the 2nd - 15 (20%) patients with chronic occlusion in anterior descending artery, the 3rd - 24 (33%) patients with chronic occlusion of circumflex branch of coronary artery. All the patients underwent complete clinical and instrumental examination including echocardiography, angiography and history taking of subjective state of patients. Subjective state of patients was assessed by an original technique three times: preoperatively, a day after admission; on the 3rd day after the operation, and 3 months after surgery. The results obtained were correlated with the effect of the disease and X-ray endovascular treatment on physical, psychological, emotional and social states of a person. Conclusion. According to the technique of subjective state assessment of patients developed by the authors, incomplete X-ray endovascular myocardial revascularization contributes to the increase of psychological and social level functioning in early postoperative periods. In accordance with the echocardiography findings, complete recovery of left ventricular myocardial function in arterial occlusive disease can fail to be observed in early postoperative periods due to collateral myocardial hypoperfusion in the zone of chronic ischemia.

Objective - to explore the sensory disturbances in the arm on the side of operation compared to the opposite upper extremity and evaluate quality of life in patients with breast cancer with postmastectomy syndrome. Material and methods. Authors examined 110 women during one year including 64 (58.2%) with intact intercostal and shoulder nerves (group 1) and 46 (41.8%) with the nerves removed due to the progression of cancer or anatomical characteristics (group 2). We studied sensory disturbances and assessed quality of life with SF-36 questionnaire. Results and conclusion. In group 1, there were less women with hyperesthesia, and quality of life level was higher compared to group 2. In group 2, the signs of hyperesthesia were more marked that may be related with the damage and/or compression of these nerves during the axillary lymphadenectomy. Preservation of intercostal and shoulder nerves during the axillary lymphadenectomy in patients with breast cancer can reduce sensory disturbances and improve quality of life of the patients. © 2015, Media Sphera. There has been demonstrated the possibility of using SilverHakw for targeted catheter atherectomy in the patient with femoral arterial occlusive disease by the example of clinical observation. The results of the performed targeted atherectomy prove the efficiency and safety of the mentioned device application in the present clinical situation. The targeted atherectomy technique enables to avoid stent implantation in complete arterial lumen recovery.
