The aim of the study is to assess antioxidant activity of Xymedon in ointments in reparative process of septic wounds of soft tissues in the experiment on dogs. Materials and methods. To treat septic wounds, in I phase of wound process in the control group of animals Laevomecolum ointment was used, in the main group - Levoxycol, containing Laevomycetin - 0.75 g, Xymedon - 4 g, polyethylene oxides with molecular weight 1500 400-76.2g. The effectiveness of treatment was assessed based on subjective (visual test of wound condition) and objective (microbiological, cytological investigations, biochemical test, planimetry) criteria. To assess oxidative process in paravulnar tissues, there were determined the activity of the enzymes of catalase and superoxide dismutase, lipid peroxidation activity (diene, triene conjugates and the number of olefinic links). Results. Oxidation-reduction processes in paravulnar tissues were characterized by maximum values of lipid peroxidation before the treatment, at the height of inflammation. In the course of treatment there was tendency for gradual results normalization and the return to their initial level, however, in the main group treated by Levoxycol the tendency was more expressed. Improved repair characteristics of ointments based on Xymedon can be explained mainly by the increase of antioxidant activity of superoxide dismutase and catalase that can be due to Xymedon as an activator of enzymes and endogenic antioxidant resulting from the process of it biotransformation.

The aim of the study is to determine the influence of intra-enteric pressure level on the mechanism causing decompensation of microcirculation defects in the wall of a small intestine and to assess the effectiveness of enteral lavage by hydroxyethyl amylum in case of an increased intra-enteric pressure in acute intestinal obstruction. Materials and Methods. The experiment was carried out on 54 laboratory animals. In I series of the experiment (36 non-pedigree dogs) the processes of microcirculation in the wall of a small intestine depending on the increase level of intra-enteric pressure (5, 15, 25, 35 mm Hg) were studied. In the II series (18 rats) there was examined the effect of 10%-hydroxyethyl amylum solution on an inflammatory process in small intestine tissues when a critical level of intra-enteric pressure was adjusted. Results. Under intra-enteric pressure in a small intestine of 15 mm Hg and more, decompensation disorders of splanchic microcirculation processes start. Under an adjusted critical level of intra-abdominal pressure, enteral lavage by 10%-hydroxyethyl amylum solution decreases the intensity of an inflammatory reaction in tissues of an intestinal wall in acute enteric obstruction.

We studied the dynamics of neurological deficit and parameters of the blood flow through middle cerebral arteries in 97 patients with acute ischemic stroke (IS). Absolute and relative parameters of the blood flow were compared to the severity of neurological deficit in the 1st, 5-7th and 21-23rd days. There was a strong correlation between the asymmetry in blood flow velocity in the first day of II and NIHSS scores in 21-23rd days (r= -0,96; p<0,01). The less pronounced asymmetry in the first day of IS predicted the better recovery of damaged functions in patients with IS.

The aim of the study is to increase the effectiveness of complex treatment of patients with acute intestinal obstruction by using new operative and instrumental technologies for prevention and treatment of infectious complications. Materials and Methods. The study is based on the analysis of the results of treatment of 132 patients operated on for acute intestinal obstruction. In the main group there have been used the developed algorithm of control of biophysical alterations of abdominal parameters and an instrumental and technical complex for atraumatic suture of laparotomy wound using the device of control of tissue tension force and intra-abdominal pressure. To match the edges of laparotomy wound and put stitches there has been used a new model of a needle abdominal wound contractor. Results. Direct correlation dependence between an intra-abdominal pressure increase and the development of systemic inflammatory response has been determined. Microbial spectrum of abdominal cavity and the role of hemolytic strains in the development of systemic inflammatory response and infectious complications have been studied. Due to the application of the developed algorithm and an instrumental and technical complex, the frequency of wound and intra-abdominal infectious complications has been reduced from 18,9 to 8,1%.

Possibilities of modern instrumental diagnosis and the differential clinical diagnosis of lumbar stenosis are presented. A modern treatment tactics and indications for surgical treatment are described. A brief and schematic description of the methods of surgical treatment of stenosis is presented.
