This study was purposed to evaluate the efficacy and safety of linezolid, vancomycin and teicoplanin for the treatment of patients infected by Gram-positive bacteria in the Department of Hematology by retrospective analysis. The patients with fever in our department from January to December in 2011 were selected for blood culture with Gram-positive bacteria and treated with linezolid, vancomycin or teicoplanin alone.Various parameters were recorded before and after treatment, such as fever time, respiratory symptoms, physical signs, radiographic changes, blood and biochemical routine, and adverse reactions. The efficacy and safety of linezolid, vancomycin and teicoplanin were compared according to the fever abating time, bacterial clearance rate, clinical efficiencies and adverse events. The patients were divided into linezolid group (15 patients), vancomycin group (17 patients) and teicoplanin group (20 patients). The results showed that the mean time of fever abating in linezolid group was (4.43 ± 3.15)d, bacterial clearance rate and clinical efficiency in linezolid group were 55.56% and 86.67%, respectively. The above three data in vancomycin group were (6.83 ± 4.67)d, 54.54% and 76.47% respectively, and were (5.57 ± 4.16)d, 41.67% and 80.00% in teicoplanin group respectively. There was no statistically significant difference between three groups (P > 0.05). There were one case of diarrhea and two cases of thrombocytopenia in the linezolid group, and one case of nausea and two cases of creatinine increase in the vancomycin group. There were three cases of thrombocytopenia in the teicoplanin group. The thrombocytopenia in five cases and the hemogram drop in patients with leukemia after treatment were overlapped, their drug treatment did not stop, but their thrombocytopoiesis recovered to normal-level, thus the drug treatment were considered as no relation with thrombocytopenia. It is concluded that the treatment efficacy between linezolid, vancomycin and teicoplanin for Gram-positive bacterial infections is not statistically different, but linezolid maybe have advantage over vancomycin and teicoplanin in fever abating time, bacterial clearance rate and clinical efficiency. Pharmacokinetics can help us to understand the metabolic feature of drugs and it also can instruct the dosage, interval, route and form of drugs used in clinic, guarantee the curative effects and reduce the side effects and toxicity. With the wide range of applications, high sensitivity and specificity, chromatography and immunoassay have been the common methods used in pharmacokinetic study, but the results are one-dimensional, operation is complicated and there are more interference factors. Imaging examination and Raman spectroscopy can be carried out in the ocular pharmacokinetic research noninvasively and continuously in vivo, but only for some specific drug testing. Subject to the restrictions of the devices themselves, specificity and sensitivity are somewhat less. Therefore, it is necessary to chose a suitable method taking into account the drug delivery methods, drug target tissue, drug metabolism characteristics of the selection of suitable methods, or combine with several methods as complementary and validation. In addition, a variety of pharmacokinetic models can also well describe, explain and predict the metabolism of drugs in eyes. Source

In the electronic endoscopic system, light source's aging problem can lower color temperature and brightness of the images, which will affect the imaging quality seriously. To make up for the color temperature, auto white-balance processing is required. By analyzing imaging characteristics, an algorithm model based on white patch theory is proposed. According to the spatial information of highlight spot, the local color temperature is established, by which way average chromaticity aberration correct factor increases from 9.03% to 79.34% compared with classical algorithm. Subsample model is used to simplify the estimation of color temperature without losing image quality, and processing time is decreased by 54.95%. The paper's automatic white balance algorithm for electronic-endoscope images can precisely restore color features for human tissues, and also inherits good real-time specialty. Source

To find out the changing trend of medical behaviors in clinical pathways (CPs) can present convincing evidence for the improvement of clinical pathway template and help to improve the medical quality of CPs. In this study, we proposed a two-step approach to detect medical behavior changes in CPs over time. In the first step, we adopted a well-known probabilistic topic model, i. e., latent Dirichlet allocation (LDA), to discover yearly treatment patterns w. r. t risk stratifications of patients from electronic medical records (EMR). With the discovered treatment patterns, the changes of medical behaviors were classified into six content change patterns (i.e., emerged, faded, stable, up-down, down-up and jumping) and three occurring time change patterns (i. e., early-occurred, stable and delay-occurred). The proposed method was evaluated via 12152 EMRS pertaining to the unstable angina pathway with time arranges of 10 years. Experimental results indicated that the proposed approach is effective to mine the significant changes of content and time of medical behaviors and therefore presents convincing evidences for scheduling better practice of CPs. The efficacy of Chinese herbal formula in treating depression has been proved in many studies. In this study, six different Kaixin San formulas were compared to investigate their effects on central monoamine neurotransmitters of chronic stress rats and against depression based on their different components in plasma, in order to discuss the efficacy-comparability relationship and the possible efficacy mechanism. The classic isolation method and the chronic unpredictable mild stress (CUMS) depression model were combined to investigate the changes in contents in hippocampus and monoamine neurotransmitters (NE, DA, 5-HT) and the components of some formulas in plasma with HPLC and UPLC-Q-TOF-MSE methods. As a result, Dingzhi Xiaowan recorded in Essential Recipes for Emergent Use Worth A Thousand significantly increased the behavioral scores, NE and 5-HT contents in hippocampus and NE, DA and 5-HT contents in cortex, with the best anti-depressant effect. Dingzhi Xiaowan recorded in Complete Records of Ancient and Modern Medical Works showed a notable increase in sucrose preference and open field score in model rats, NE content in hippocampus and NE, DA and 5-HT contents in cortex, with a certain anti anti-depressant effect. Kaixin San recorded in Ishinpo showed remarkable rise in weight of model rats, NE content in hippocampus and DA content in cortex. Puxin Decoction recorded in A Supplement to Recipes Worth A Thousand Gold showed 5-HT content in hippocampus and DA content in cortex. Kaixin San recorded in Yimenfang only showed DA content in cortex. Kaixin Wan recorded in Essential Recipes for Emergent Use Worth A Thousand did not mention the antidepressant effect. According to the results, the formulas' different anti-depressant effects may be related to the different plasma components. Source
