This study investigated the role of drug-induced sleep endoscopy (DISE) in the surgical treatment planning of patients with obstructive sleep apnea syndrome (OSAS). This study was conducted using patients diagnosed with OSAS between January 2007 and March 2009, who were scheduled for surgical treatment. DISE was performed using propofol in patients considered to have upper respiratory tract obstruction as indicated by Muller’s maneuver. After completing the sleep endoscopy, the patient was intubated and surgery was performed (tonsillectomy and uvulopalatopharyngoplasty). A successful operation was defined as a decrease in the respiratory disturbance index to below 5 or a decrease of ≥50 % following the operation. The study included 20 patients (4 female and 16 male) aged 19–57 years. No statistically significant correlation between modified Mallampati class and operation success or between the polysomnographic stage of disease and operation success was identified. A significantly high operation success rate was found in the group with obstruction of the upper airway according to DISE (p < 0.05), whereas a significantly low operation success rate was found in the group with obstruction of the lower airway according to DISE (p < 0.01). DISE may be used to identify the localization of obstruction for diagnostic purposes, and it can be helpful in selecting the treatment method. © 2014, Springer-Verlag Berlin Heidelberg. Source

A wide variety of autologous grafts have been used for the coverage of contour deformities in rhinoplasty. Mastoid fascia tissue (MFT) grafts are easy to harvest and very versatile, and the resulting donor site scar is not visible. Natural looking results can be obtained with MFT grafts when used for the coverage of dorsal irregularities, for dorsal augmentation, for camouflaging abnormalities related to skin atrophy, and even for tip-of-nose coverage and tip augmentation. Mastoid fascia tissue was used as a graft in 8 primary and 22 secondary cases for various purposes. All patients had satisfactory cosmetic results. Used in conjunction with a rigid graft such as cartilage, MFT grafts, which are ideal fillers, can be successfully used in rhinoplasty. Copyright © 2012 by Mutaz B. Habal, MD. Source

Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. For these reason we found important to report this case of CCOC in the maxilla. Source

Background Cognitive impairments and subsyndromal depressive symptoms are present during euthymic periods of bipolar disorder (BD). Most studies have determined that cognitive impairments and residual depressive symptoms have major impacts on psychosocial functioning. The aim of the present study was to identify the major factor responsible for low psychosocial functioning in a subgroup of patients with BD despite clinical recovery.Methods Sixty patients with bipolar I disorder and 41 healthy subjects were enrolled in this study. Cognitive performance, neurological soft signs (NSSs), psychosocial functioning, residual mood symptoms and illness characteristics were assessed. Using the median value of the Functioning Assessment Short Test (FAST) as the cut-off point, the patients were divided into two groups, high- (n=29) or low-functioning (n=31), and they were compared based on total NSS, residual depressive symptoms, cognitive performance and clinical variables.Results Performances on the verbal memory tests and social functioning were significantly worse in the euthymic patients with BD. Increased rates of NSS were identified in the patients compared with the normal controls. The low-functioning patients performed significantly worse on verbal memory, and their NSS and residual depressive symptoms were significantly higher compared to high-functioning patients. In the regression analysis, subsyndromal depressive symptoms and verbal learning measures were identified as the best predictors of psychosocial functioning.Limitations The patients were artificially separated into two groups based on a FAST score cut-off.Conclusions In this study, residual depressive symptoms and verbal memory impairments were the most prominent factors associated with the level of functioning. © 2014 Elsevier B.V. A 39-year-old otherwise healthy barber presented to our clinic complaining of frequent periods of swelling and purulent discharge in his second and third right web spaces. An excision was performed after administration of methylene blue to visualize the extent of both cysts located in his second and third web spaces. A distally based dorsal metacarpal artery perforator flap was chosen to close the defect on his second web space. Interdigital pilonidal sinus is a rare disease caused by repeated implantation of foreign hair to the interdigital web space. In secondary cases or in cases where an extensive resection has to be done in order to completely remove the cysts, closure with a skin flap can become compulsory. Dorsal metacarpal artery perforator flap is a good choice with minimal donor site morbidity and this flap might provide robust skin coverage which can possibly avoid further penetration of hair in to web space. © 2009 British Association of Plastic, Reconstructive and Aesthetic Surgeons.
