Results of creation of mathematical model for postoperative treatment duration forecasting at performance of hi-tech operations on heart on elimination of arrhythmias are presented in the article. To identify characteristics of development of human pancreatic stroma in the fetal and neonatal periods of ontogenesis, we have conducted a cross prospective study of 208 pancreases of fetuses (16-40 weeks) and dead newborns. They have been divided into groups by age. Data were processed by methods of nonparametric statistics. It has been established that in pathological pregnancies, formation of the pancreas stroma components was interrupted. A reliable connection between collagen content and perinatal pathology development risk (rs = 0.145, p = 0.001) has been detected, as well as in the groups "collagen - Age" and "collagen - cause of death" (rs = -0.184, p = 0.003; rs = -0.194, p = 0.005). In the group of cases caused by separate states, the content of collagen was significantly higher (p = 0.024). Conclusions: it is necessary to consider the risk of development of the perinatal pathology as one of the major factors causing overdevelopment of the pancreas stroma collagen, what reflects environmental impacts on the feto-placentary system. Link the degree of progression of motor and cognitive impairment in Parkinson's disease and parameters of cognitive evoked potentials brain activity recognized not always unambiguous. This is due to the wide range of differences in amplitude and latency time of evoked potentials in the different leads of electroencephalogram caused functional reorganization of neuronal activity in brain structures in the early stages of the disease. Objective. The aim is to determine the correlation of the cognitive auditory evoked potentials P300 characteristics and symptom features in patients with Parkinson's disease. Methods. The study involved 60 people, right-handers: 34 people (24 women and 10 men) with a verified diagnosis of Parkinson's disease and the control group - 26 people (20 women and 6 men). The disease duration, severity of symptoms (Unified Parkinson's Disease Rating Scale), and stage of the disease (Hoehn, Yahr) were determined, the 15-item Geriatric Depression Scale was used to indicate the depression symptoms. Assessment of the electroencephalogram and auditory evoked potentials P300 was performed by an electroencephalograph «Encephalan» (Medicom, Taganrog, Russia). The minimum amplitude and the maximum latency of P300 in electroencephalogram-leads (F3, F4, C3, C4, P3, P4, F7, F8, T3, T4, T5 and T6) on the right and left sides were determined. Results. A significant positive correlation between the amplitude of evoked potential P300 on the left and duration of Parkinson's disease was revealed, moreover the severity of disease symptoms such as left-side resting tremors of the hand, left-side muscle tension of the hand and/or foot, rising from a chair without using their hands, posture disorder were established. Conclusion. Marked brain asymmetry in the form of increase of the P300 amplitude on the left side combined with a significant reduction in the P300 amplitude on the right side in contra lateral electroencephalogram-lead indicates unfavorable prognosis in relation to cognitive dysfunction and motor disorders identified with left-sided Parkinson's disease onset. Objectives. Off pump beating heart technique for myocardial revascularization was used successfully both in low and high risk patients. Its application in low ejection fraction patients was limited. The aim of our study was to evaluate the results of this technique application in patients with low ejection fraction and to compare it with the on pump bypass technique. Methods. This retrospective study included 174 patients with ejection fraction below 0.45 who underwent isolated coronary artery bypass surgery. 37 patients underwent myocardial revascularization with the use of conventional coronary artery bypass grafting (cCABG), while 137 patients had off-pump beating heart bypass grafting (OPCAB). Different preoperative, operative and postoperative variables were evaluated in both groups. Results. The patients profiles and risk factors were similar in both groups. The mean EF for patients who underwent cCABG was 43 % (Q1 = 40; Q3 = 44) in comparison with 42 % (Q1 = 38; Q3 = 44) for the OPCAB patients - (U = 2105,5; Z = -1,6; p = 0,11). Predicted risk for surgery according to EuroSCORE was similar in both groups (U = 2004.0; Z = -1.97; p = 0.05). The number of grafts performed per patient was significantly bigger in the patients who underwent cCABG - 4.0 (Q1 = 3.0; Q3 = 4.0) Vs 3.0 (Q1 = 3.0; Q3 = 4.0) in OPCAB; (U = 1625.5; Z = -3.55; p < 0.001). Completeness of revascularization was similar in both groups (U = 2335.5; Z = -1.0; p = 0.316). Incidence of hospital mortality and complications were higher in cCABG in comparison with OPCAB. Incidence of inotropic support, ICU, hospital stay and blood transfusion was significantly bigger among patients who underwent cCABG. Conclusions. OPCAB technique can be used in myocardial revascularization in patients with left ventricular dysfunction. Morbidity and mortality was lower than that in the cCABG group. Source
